吸入普米克令舒对先心病小儿围术期肺功能的影响

摘　　要：【目的】观察先心病小儿体外循环心脏直视术前短期吸入糖皮质激素普米克令舒（布地奈德）对围手术期肺功能的影响。【方法]30例拟在全身麻醉体外循环下行心脏直视手术的房间隔缺损和空间隔缺损缺惠儿随机分为激素吸入组及对照组，每组15例，吸入组惠儿术前给予普米克令舒lmg／2ml雾化吸入，对照组则雾化吸入等容积生理盐水。监测并记录手术前（吸入前）、开胸前（吸入后）、体外循环前、体外循环停机后和关闭胸腔后的肺顺应性（Compl）、气道阻力（Raw）、气道峰压（Ppeak），气道平台压（Pplat）和气道平均压（Pmean）。【结果】普米克令舒吸入组患儿术后的Compl、Raw、Ppeak、Pplat和Pmean与对照组相比无统计学差异（P〉0．05）。【结论】手术前短期雾化吸入糖皮质激素普米克令舒对婴幼儿心脏直视术围术期肺功能无显著改善。[Objective]To observe the effect of short-term corticosteroid（pulmicort respules） inhalation before operation on perioperative lung function in pediatric patients with congenital heart diseases undergoing open heart surgery with cardiopulmonary bypass（CPB）. [Methods] Thirty pediatric patients with atrial septal defect and ventricular septal defect undergoing open heart surgery with CPB were randomly divided into corticosteroid inhalation group（ n = 15） and control group（ n =15）. The pediatric patients in inhalation group were given aerosol inhalation with pulmicort respules lmg/2ml before the operation. The pediatric patients in control group were given aerosol inhalation with equal volume of normal saline. Lung compliance（Compl）, airway resistance（Raw）, airway peak pressure （Ppeak）, plat pressure（Pplat） and mean pressure（Pmean） were monitored and recorded before operation（before inhalation）, thoracotomy（after inhalation） and CPB and after CPB and chest closure. [Results]There was no significant difference in Compl, Raw, Ppeak, Pplat and Pmean between pulmicort respules inhalation group and control group（ P〉0.05）. [Conclusion] Short-term（10min） corticosteroid（pulmicort respules） inhalation before operation can not improve perioperative lung function in pediatric patients undergoing open heart surgery with CPB.
