摘　　要：目的:探讨颈部扩张皮瓣用于下面部瘢痕修复的效果。方法:将扩张器置于颈部扩张3-5个月,然后,将扩张的皮瓣向头侧铺展来修复下面部瘢痕包括上唇瘢痕。在扩张皮瓣上对应口的位置开个洞以修复口周瘢痕。结果:从2004年1月至2009年1月,我们用颈部扩张皮瓣修复下面部瘢痕患者8例。有6例患者扩张皮瓣存活良好,有2例患者,扩张皮瓣的远侧边缘有小范围坏死,大部分的扩张皮瓣存活良好,伤口通过换药愈合。所有病人都获得满意结果。平均随访的时间是13个月。扩张皮瓣与周围皮肤在色泽、纹理以及厚度上都很相似。扩张皮瓣移植后未发生挛缩现象,颌颈角外形满意。下唇外翻及流涎得到良好矫正。结论:对于下面部瘢痕且颈部良好的患者,整个颈部的扩张皮瓣是修复下面部烧伤后瘢痕良好选择。Objective：To investigate the effect of using pre-expanded flap in the reparation of lower face scar.Method：Tissue expanders were implanted in the neck and inflated for 3 to 5 months.Then the expanded neck flap was advanced to the cephalic side to resurface the scar of lower face including the upper lip.A hole was made in the flap corresponding to the position of the mouse.Result：From January 2004 to January 2009,we used pre-expanded flap of the whole neck to resurface the post-burn scar of lower face in 8 patients.Flaps survived well in 6 patients.The majority of flap survived with only small necrosis in the distal edge in 2 patients.The wound healed with dress changing.All patients got satisfactory outcome.The period of follow-up was 13 months in average,The flaps matched well to the surrounding skin in terms of color,texture and thickness.No contracture of the flap occurred,and the mental cervical angle had good figure.The ectropion of the lower lip and drooling were rectified.Conclusion：In the patients with lower face scar,and with sufficient unharmed neck flap.The expanded flap of the whole neck is suitable to be used to resurface the burn scar in the lower face.
