两种无肝素法CVVH对成年凝血机制障碍患者血清电解质的影响

摘　　要：目的观察无肝素法连续静脉-静脉血液滤过(CVVH)中两种管道冲洗液对出血或/和凝血机制障碍患者血清电解质的影响,为患者寻求一种更合适的治疗方案。方法 78例行CVVH的患者,随机分为A组和B组,每组39例,A组用置换液做冲洗液,B组用生理盐水做冲洗液,测患者治疗前、治疗后2h、4h、6h的血清电解质,将治疗后2h、4h、6h血清电解质的值与治疗前做比较。结果 A组治疗后2h、4h、6h电解质(K+、Na+、Mg2+、Ca2+)较治疗前无明显变化;B组治疗后2h、4h、6h电解质(K+、Mg2+、Ca2+)较治疗前明显偏低。结论 A组治疗方案对患者血清电解质无明显影响,对出血或/和凝血机制障碍成年患者疗效较B组为优。Objective To observe the effects of two kinds of no heparin continuous intravenous-intravenous blood hemofiltration（CVVH） on the level of serum electrolyte in adult patients with disturbances of bleeding or blood coagulation,in order to seek a more suitable therapeutic regimen. Methods 78 patients who had been done CVVH,were divided into two average groups（A and B） randomly,and patients in group A used displacement liquid as rinse solution,while patients in group A used sodium chloride. And then,the levels of serum electrolyte at 2,4,6 hours after therapy were detected and compared with that before treatment. Results Compared with that before treatment,there was not significant change of the level of serum electrolyte（K＋、Na＋、Mg2＋、Ca2＋） from patients in group A at 2,4,6 hours after therapy,but there was significant change of the level of serum electrolyte（K＋、Na＋、Mg2＋、Ca2＋） from patients in group A at 2,4,6 hours after therapy. Conclusion Using displacement liquid as rinse solution in the process of no heparin CVVH,does not affect the level of serum electrolyte in adult patients with disturbances of bleeding or blood coagulation.
