消癥通痹胶囊治疗糖尿病下肢血管病变60例临床观察

摘　　要：目的探讨消癥通痹胶囊治疗糖尿病下肢血管病变的临床疗效。方法将糖尿病下肢血管病变120例随机分为2组,对照组予胰激肽原酶肠溶片2片,每日3次口服。治疗组予消癥通痹胶囊4粒,每日3次口服。2组均以2个月为1个疗程。观察2组治疗前后临床症状、体征变化,检测治疗前后空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbAlc)血脂[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDC-C)、高密度脂蛋白胆固醇(HDL-C)]、踝肱指数(ABI)、双侧足背动脉血流动力学指标(血管内径、血流速度、血流量)的变化。结果治疗组总有效率85.00%,对照组总有效率65.51%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。2组治疗后ABI、足背动脉血管内径、血流速度、血流量与本组治疗前比较差异均有统计学意义(P<0.05),ABI、足背动脉血管内径、血流量组间比较差异有统计学意义(P<0.05),治疗组改善优于对照组。2组治疗后FPG、2 hPG、HbAlc、TC、TG、LDL-C、HDL-C与本组治疗前比较差异均有统计学意义(P<0.05),但组间比较差异无统计学意义(P>0.05)。结论消癥通痹胶囊可通过扩张足背动脉血管内径,增加足背动脉血管的血流量,提高ABI比值,达到治疗糖尿病下肢血管病变目的。Objective To study the clinical efficacy of Xiazhengtongbi capsule on the treatment of diabetic patients with lower extremity vascular lesions.Methods 120 patients with diabetic lower extremity vascular disease were randomly divided into two groups.The control group would be given pancreatic kininogenase enteric-coated tablet 2 pills,thrice a day.The treatment group would be given Xiazhengtongbi capsule 4 pills,thrice a day.The changes of clinical symptom were observed before and after the treatment.FPG,2 hPG,blood lipid,the ankle-brachial index（ABI） and the hemodynamic indicators of bilateral dorsalis pedis artery（blood vessel diameter,blood flow,blood flow volume） were detected.Results The total effective rate of treatment group was 85.00%,which was better than the control group（65.51%）（P0.05）.There was significant difference between before and after treatment on the ABI,blood vessel diameter,blood flow and blood flow volume in two groups（P0.05）.However the effect of treatment group is more obvious than that of the control group（P0.05）.There was significant difference between before and after treatment on FPG,2 hPG,HbAlc,TC,TG,LDL-C and HDL-C（P0.05）,but there was no difference between the two groups（P0.05）.Conclusion Xiazhengtongbi capsule can effectively treat diabetic patients with lower extremity vascular lesions by expanding vascular diameter of arteria dorsalis pedis,raising blood flow of arteria dorsalis pedis and increasing ABI.
