中医分时辨治糖尿病合并抑郁症46例临床观察

摘　　要：目的观察中医分时辨治糖尿病合并抑郁症的临床疗效。方法将127例糖尿病合并抑郁症患者随机分为3组。中药组46例寅时服用调气降糖丸,巳时服用益脾降糖丸,酉时服用糖肾康;西药组41例予盐酸氟西汀胶囊口服治疗;基础治疗组40例仅维持原糖尿病基础降糖药物治疗不变。3组均治疗12周,分别观察3组治疗前后汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分、中医症状改善情况,检测血糖[空腹血糖(FPG)、餐后2 h血糖(2 h PG)]、糖化血红蛋白(HbAlc)及血脂[甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)]。结果中药组、西药组治疗后HAMD、HAMA量表积分均较本组治疗前下降(P<0.05);中药组、西药组治疗后HAMD、HAMA量表积分均较基础治疗组下降(P<0.05,P<0.01);中药组治疗后HAMD、HAMA量表积分较西药组下降明显(P<0.01)。中药组、西药组FPG、2 h PG、HbAlc、TG、TC均较本组治疗前降低(P<0.05,P<0.01),HDL-C升高(P<0.05,P<0.01);基础治疗组治疗后FPG、2 h PG、HbAlc均较本组治疗前升高(P<0.05)。中药组、西药组治疗后FPG、2 h PG、HbAlc、TG、TC均较基础治疗组降低(P<0.05,P<0.01),HDL-C升高(P<0.01)。中药组治疗后FPG、TG、TC均较西药组降低(P<0.05,P<0.01),HDL-C升高(P<0.01)。中药组除焦虑抑郁、不寐外,其他中医症状均较西药组改善明显(P<0.05,P<0.01);中药组、西药组中医症状均较基础治疗组改善(P<0.01)。结论中医分时辨治糖尿病合并抑郁症较西药可有效改善患者的血糖、血脂,降低HAMD、HAMA量表积分。Objective To observe the clinical effect of time-sharing differentiation treatment of traditional Chinese medicine on diabetes mellitus combined with depressive disorder.Methods 127 patients with depression diabetes were randomly divided into 3 groups.46 cases in traditional Chinese medicine group took Tiaoqijiangtang pill at the third double-hour,Yipijiangtang pill at the sixth double-hour and Tangshenkang at the tenth double-hour.41 cases in western medicine group orally took fluoxetine hydrochloride.40 patients with diabetes in basic treatment group only maintain the original basis of hypoglycemic drug therapy of diabetes.The course was 12 weeks in three groups.3 groups were assessed before and after treatment Hamilton Depression Rating Scale（HAMD）,Hamilton Anxiety Scale（HAMA）,improvement of clinical symptoms,blood glucose[fasting plasma glucose（FPG）,postprandial 2 h blood glucose（2 h PG）],glycated hemoglobin（HbAlc） and blood lipid[triglyceride（TG）,total cholesterol（TC）,high density lipoprotein cholesterol（HDL-C）].Results HAMD,HAMA scale score in traditional Chinese medicine group and western medicine group after treatment was reduced compared with that before treatment（P0.05）.HAMD,HAMA scale score in traditional Chinese medicine group and western medicine group after treatment was reduced compared with that basic treatment group（P0.05,P0.01）.FPG,2 h PG,HbAlc,TG and TC in traditional Chinese medicine group and western medicine group after treatment was reduced compared with that before treatment（P0.05,P0.01）.HDL-C was increased（P0.05,P0.01）.FPG,2 h PG and HbAlc in basic treatment group after treatment was increased compared with those before treatment（P0.05）.FPG,2 h PG,HbAlc,TG and TC in traditional Chinese medicine group and western medicine group after treatment was reduced compared with those in basic treatment group after treatment（P0.05,P0.01）.HDL-C was increased（P0.01）.FPG,TG and TC in traditional Chinese medicine group was reduced compared with
