摘  要:目的探讨MR弥散加权成像及表观弥散系数在乳腺病变诊断中的价值。方法对56例64个病灶经病理证实的乳腺疾病患者行磁共振弥散加权成像检查。DW-MRI采用单次激发平面回波成像技术,弥散敏感系数取b=800 s/mm2。比较良、恶性病变、正常腺体间ADC值差异的统计学意义。确定恶性病变的ADC阈值并用于病例诊断。结果磁共振的DWI对乳腺病变的显示良好。在b=800 s/mm2时,正常腺体、良、恶性肿瘤的ADC值差异有显著性(P<0.05)。恶性病变平均ADC值为(1.12±0.21)×10?3mm2/s,95%可信区间为(0.854,1.106)×10?3mm2/s。把恶性肿瘤95%可信区间上界1.106×10?3mm2/s定为良恶性病变ADC值的阈值,诊断敏感性为91.7%,特异性为85.7%。结论 DWI对良恶性病变的诊断有较高的敏感性和特异性,推荐作为常规检查序列之。ADC值是诊断乳腺病变有价值的参数。Objective: To evaluate the value of apparent diffusion coefficient(ADC) of diffusion weighted MR imaging in diagnosing breast cancer.Methods: Fifty-six patients were enrolled in this study and verified by histopathology.The apparent diffusion coefficient(ADC) of malignant and benign lesions on DW-MRI were measured respectively,and their differentiations in statistics were compared.Results: There were significant differences in the ADC values of normal breast gland,benign and malignant lesions.The ADC of malignant lesions[(1.12±0.21)×10-3 mm2/s] was lower than those of normal breast gland and benign lesions.The 95% confidence interval for ADC means in malignant lesions:(0.754,1.106)×10-3 mm2/s respectively.If the upper bound of 95% confidence interval in malignant lesion was set as the Cut off value,the sensitivity,specificity of ADC value was 1.106×10-3 mm2/s for the diagnosis of malignant lesions were 91.7% and 85.7%.Conclusion: The sensitivity and specificity of DWI is high,which is helpful for the diagnosis of benign and malignant breast lesions and can be used as a conventional sequence.ADC value is an important parameter to diagnose breast lesions.
