摘　　要：目的总结经脐单孔腹腔镜的手术经验及适应症。方法回顾性分析我院2009年11月至2010年11月43例经脐单孔腹腔镜手术病人的临床资料。结果 42例行单孔腹腔镜胆囊切除术手术成功,1例中转为3孔腹腔镜胆囊切除术。42例平均手术时间28～115(58.5±23.7)min,平均术中出血量3～70(15.3±8.6)mL,术后平均住院2.3(2～4)d,1例术后出现切口疝,1例出现切口脂肪液化,经换药后愈合外,其余病例伤口愈合良好,瘢痕隐蔽不易发现,美容效果满意。随访5～19个月,无胆漏、出血、胆管损伤等严重并发症。结论经脐单孔腹腔镜胆囊切除术安全可行,具有"无瘢痕"、恢复快等优势,但是对技术要求较高,必须严格选择病例,设备不断改进,对病人有选择的进行。Objective To summarize the operative experience and indications of transumbilical single-port laparoscopic cholecystectomy. Methods From November 2009 to Dec 2009, the clicinal data of 43 cases received transumbilical single-port laparoscopic cholecystectomy were analyzed retrospectively. Results The transumbilical single-port laparoscopic cholecystectomy were performed successfully in 42 cases. One case was transferred to 3-ports laparoscopic cholecystectomy because of severe adhesion of peri-gallbadder. The mean operative time was 58.5±23.7 minutes （28-115 minutes）. The mean volume of intraoperative blood loss was 15.3±8.6ml （3-70ml）. The mean postoperative hospitalization time was 2.3 days （2-4 days）. Postoperative complications： one case occurred incisional hernia and 1 case occurred fat liquefaction of incisional. No biliary leakage, bleeding and bile duct injury were found after operation. Conclusions Transumbilical single-port laparoscopic eholecystectomy has safe and feasible, no scar and quick recovery etc. advantages. But a comparative high technique is necessary to this operation and the cases must be selected strictly.
