Bassini法结合腹横筋膜修补和联合腱下移治疗腹股沟疝51例

摘　　要：目的总结Bassini法结合腹横筋膜修补和腹直肌前鞘切开减张治疗腹股沟疝的临床经验。方法在采用Bassini方法修复腹股沟疝的手术中,首先小跨距间断折叠缝合腹横筋膜以缩小内环口;然后在腹直肌前鞘下段作一弧形减张切口,使联合腱向下、外侧移位,于精索或子宫圆韧带后方在无张力状态下将联合腱与腹股沟韧带间断缝合。结果采用该方法治疗腹股沟疝51例,患者术后手术部位无明显牵拉不适感,阴囊或腹股沟区无水肿发生。平均住院时间6.8天。术后获得随访42例,随访率为82.4%;随访时间5个月至3年,中位随访时间为1.5年,无疝复发病例。结论该术式操作简单,疗效好、复发率低,是较好的疝修补方法之一。Objective To summarize the clinical experience in use of Bassinis procedure combined with fascia trans versalis and moving down conjoined tendon for treating inguinal hernia. Methods In the Bassinis operation for the treatment of inguinal hernia,first of all, interrupted suture of the fascia trans versalis was performed to shrink the orifice of internal ring. And then , a arcate decompressive incision was done on the low segment of anterior sheath of rectus abdominis,which conjoined tendon was displaced down and laterally. The conjoined tendon and inguinal ligament were sutured by interrupted suture behind the spernlatic cord or round ligament of uterus in the state of tension-free. Results In this group of 51 cases, no signicant postoperative complications were found including dragging pain local hematoma, edema of scotum or inguinal area, incisional ingeetion and so on. The average hospitalization was 6.8 days. Forty-two cases （82.4%） were followed up and ranged from 5 to 36 months. There was no case of hernia recurrence. Conclusion This surgical procedure has many advantages,such as easy manipulation,mild postoperative pain,quick recovery and low recurrence rate. Therefore, it is a better surgical procedure for the treatment of inguinal hernia.
