彩色多普勒超声检查对诊断糖尿病下肢血管病变的临床价值

摘　　要：目的探讨彩色多普勒超声检查在糖尿病下肢血管病变中的诊断价值。方法用彩色多普勒超声检查36例糖尿病足患者双下肢动脉血管内膜、斑块、狭窄及闭塞情况,对其中9例糖尿病足坏疽患者同时作下肢动脉数字减影血管造影(DSA)。结果彩色多普勒检查发现糖尿病下肢血管病变主要位于腘动脉以下,呈多节段性改变,坏疽组病变较非坏疽组严重,与DSA结果比较,彩色多普勒在诊断糖尿病下肢血管病变中的符合率在85%以上。结论临床作为无创性检查彩色多普勒可代替DSA诊断糖尿病下肢血管病变。Objective To study the value of diabetic vascular lesion of lower limbs with color Doppler imaging.Methods We observed the lower extremity arterials vascular intima,plaque,narrow and occlusion in 36 patients with diabetic foot with color Doppler imaging.Digital subtraction angiography（DSA） with 9 of diabetic foot gangrene was observed.Results Vascular lesions was mainly below popliteal arteries and lesions was multi-segmental.The patients with diabetic foot gangrene was more serious than the patients with diabetic foot non-gangrene.Compared with DSA,color Doppler imaging in diagnosis of diabetes vascular lesions in the lower extremities were much accurate.Conclusion Compared with DSA,color Doppler can replace DSA in the diagnosis of diabetic lower extremity vascular disease.
