血塞通注射液与氯沙坦联合治疗早期糖尿病肾病疗效观察

摘　　要：目的探讨血塞通注射液与氯沙坦联合治疗早期糖尿病肾病的疗效。方法选择早期糖尿病肾病患者120例。随机分为两组,对照组:常规治疗+口服氯沙坦50mg/d,每日1次;实验组:常规治疗+口服氯沙坦50 mg/d,250 ml生理盐水+血塞通注射液0.45 g,静滴每日1次。两组均为15 d 1个疗程,停药5 d后开始第2疗程,共2个疗程。两组患者治疗前后测定24小时尿白蛋白排泄率(UAE)、白蛋白(ALB)、血胆固醇(CH0)、甘油三酯(TG)、TL高密度脂蛋白(HDL)、24 h尿蛋白定量、静息血压(BP)、空腹血糖(FBS)、餐后2 h血糖(PBG)、血液流变学中全血高切还原黏度、全血低切还原黏度、红细胞聚集指数(EAI)。结果实验组较对照组可以更好的降低早期糖尿病肾病UAE,升高ALB(P<0.01),也可降低CHO、TG及升高HDL,改善血液流变学(P<0.05)。结论血塞通注射液与氯沙坦联合应用治疗早期糖尿病肾病,更有助于改善肾小球滤过率,减少尿蛋白,疗效好于单用氯沙坦。Objective To explore the therapeutic effectiveness of Sanqi Panaxnotoginseng injection combined with losartan in treating the early diabetic nephropathy.Methods 120 early diabetic nephropathy patients were randomly divided into two groups,including control group treated with conventional hypoglycemic drugs and losartan（50mg qd） and treatment group treated with conventional hypoglycemic drugs and losartan（50mg qd） and 250ml normal saline and 0.45g Sanqi Panaxnotoginseng injection by intravenous drip one time a day.Before and after the treatment,the following index of the two groups were measured： urinary albumin excretion rate of 24hours（UAE）,albumin（ALB）,blood cholesterol（CHO）,triglycerides（TG）,TL high density lipoprotein（HDL）,24 hours urine protein quantitative,venous blood pressure,fasting blood sugar（FBS）,postprandial blood glycemia of two hours（PBG）,whole blood high cutting reductive viscosity of Blood rheology,whole blood low cutting reductive viscosity,erythrocyte aggregation index（EAI）.Results The UAE were reduced and the level of ALB were raised in treatment group compared with that of control group（P0.01）.The CHO and TG were reduced,HDL were elevated and hemorheology were improved in treatment group.Conclusion Sanqi Panaxnotoginseng injection combined with losartan can improve the glomerular filtration rate and reduce urinary protein in treating with the early diabetic nephropathy.The effectiveness of the combined therapy is better than the separated treatment of losartan.
