超声引导下改良经肘上外周穿刺置入中心静脉导管的效果观察

摘　　要：目的探讨超声引导下改良经肘上外周穿刺置入中心静脉导管(peripherally inserted central venous catheters.PICC)的应用效果。方法将92例需行PICC置管但血管条件差的患者随机分为观察组和对照组,对照组行常规PICC,观察组在超声引导下经肘上3 cm处行PICC。比较两组患者的置管成功率、并发症(血栓形成、穿刺部位渗血、脱管堵管和机械性静脉炎)的发生情况以及患者的满意度、舒适度。结果两组患者置管成功率、相关并发症总发生率、患者置管满意度和舒适度方面的差异具有统计学意义(均P<0.05)。其中观察组置管成功率为100%(40/40),高于对照组的88%(46/52);并发症总发生率为5%(2/40),低于对照组的34.7%(18/52);满意度和舒适度分别为92.5%(37/40)、85.0%(34/40),高于对照组的57.7%(30/52)、48.1%(25/52)。结论与常规PICC相比,超声引导下经肘上外周穿刺中心静脉置管成功率高,患者并发症少,提高了患者满意度和舒适度。Objective To explore the effectiveness of peripherally inserted central venous catheters（PICC） guided by ultrasound above elbow.Methods 92 patients C were randomly divided into two groups.52 patients in the control group received routine PICC,while 40 patients in the experimental group received PICC guided by ultrasound above elbow.The success rate of catheter placement,the incidence of complications,the patients＇satisfaction degree and the degree of comfort were compared between the two groups.Results Significant differences were found in the success rate of catheter placement,the incidence of complications,patients＇ satisfaction degree and degree of comfort（all P0.05）.The success rate of catheter placement was 100%in the experimental group and 88% in the control group.The total incidence of complications was 5% in the experimental group and 34.7%in the control group.Patients＇satisfaction rate and degree of comfort were 92.5%（37/40）、 85.0%（34/40） respectively in the experimental group and were 57.7%（30/52）,48.1%（25/52）respectively in the control group.Conclusion Compared with routine PICC,PICC guided by ultrasound above elbow can improve the success rate of catheter placement,patients＇satisfaction degree and degree of comfort,reduce the incidence of complications.
