港冀精神科防暴约束用具约束效果的对照研究

摘　　要：目的了解香港及河北省两地精神科护士应对精神科暴力行为时所使用的约束用具（德国磁扣式约束用具和传统的约束带）的约束效果及差异，为河北省精神科临床约束用具的更新换代提供依据。方法本研究采用自制调查问卷，以香港和河北省的部分精神疾病专科医院的护理人员为调查群体，对两地精神科约束工具的约束效果及差异进行对照分析。结果（1）约束用具是否科学实用，港冀两地差异有统计学意义（χ2=222．85，P〈0．001），被约束的患者舒适程度的比较差异有统计学意义（χ2=30．44，P〈0．001）；（2）约束用具的约束效果比较，包括是否发生约束带脱出、约束带脱出约束失败造成暴力行为、约束效果及防止约束带脱出的效果港冀两地比较差异均有统计学意义（χ2=56．45、29．55、206．15和206．15，P〈0．001）；（3）约束用具达到约束效果需约束的部位，港冀两地差异有统计学意义（χ2=70．14，P〈0．001；（4）港冀两地精神科护士对目前所使用的约束用具不满意希望更新约束用具的需求情况，港冀两地差异有统计学意义（χ2=106．90和140．14，P〈0．001）。结论河北精神科目前所使用的约束工具（传统的约束带）无论是科学实用性、约束效果、要达到更好的约束效果所需约束患者的部位、被约束患者的舒适度，还是护士对科学实用的约束用具的需求情况，与香港精神科目前所使用的约束工具（德国磁扣式约束工具）都存在较大差异，香港目前所用的约束工具约束效果好、被约束患者舒适度高，更加科学实用。所以，为提高精神科临床护理约束用具的约束效果，更好地保护患者和工作人员，应尽快为河北省精神科临床更新更加科学、实用、约束效果更好的新一代约束工具，减少因约束工具落后、约束效果差而造成患者和护士的�Objective To better understand the effects and differences of anti--riot constraint equipments used by psychiatric nurses between Hong Kong and Hebei,therefore to provide a reference for updating the constraint equipments in Hebei Province. Methods A self--designed questionnaire was developed and distributed to psychiatric nurses working in various mental hospitals/units in Hebei Province and Hong Kong. The results were then compared and analyzed. Results （1）The scientific and practical aspects of the constraint equipment were significantly superior in Hong Kong（ χ2= 222.85,P 〈 0. 001）. The patients were significantly more comfortable in Hong Kong（ χ2= 30.44,P〈 0. 001）. （2）The effect of constraint equipment was significantly better in Hong Kong（ χ2= 56.45,29.55,206.15 and 206.15, P 〈0. 001）. （3）The constraint site was more significantly reasonable in Hong Kong（ χ2= = 70.14,P 0. 001）. （4）The psychiatric nurses were more hopeful for the update of the constraint equipment in Hebei province（ χ2= =106.90 and 140.14, P 〈 0. 001）. Conclusions The constraint equipments of Hong Kong are better in the following areas, the scientific and practical measures, the effectiveness, the reasonable constraint site, the degree of comfort. Consequently, in order to improve the effectiveness of constraint equipment and safety for patients and the staff, we should update and apply more scientific and practical equipment, so as to decrease the harm to patients and staff because of backward equipment.
