摘  要:目的探讨磁敏感加权成像(SWI)在新生儿缺氧缺血性脑病(HIE)诊断中的价值。方法选取HIE新生儿60例,采用德国Siemens公司Avanto 1.5T超导型MR扫描仪,检查前30 min给予10%水合氯醛口服或灌肠(0.3~0.5 ml/kg),使患儿在睡眠状态下完成检查。常规行横轴位T1WI、T2WI和DWI扫描,均增加横轴位SWI扫描。比较头颅常规MRI序列与SWI序列检查对病灶的检出率,部位及性质。结果 SWI检出的脑微出血病灶数量最多,与T1WI、T2WI、DWI检出率比较,差异均具有统计学意义(均<0.01),在病灶大小、范围及边界清晰度显示方面,SWI均明显优于T1WI、T2WI和DWI。结论 SWI在HIE微小出血灶的数量及面积的显示上优于常规序列,具有明显优势,有助于更好地评估HIE损害的程度及预后,具有一定的推广价值。Objective To explore the diagnostic values of usceptibility weighted imaging(SWI)on hypoxic-ischemic encephalopathy(HIE).MethodsSiemens Avanto 1.5T superconducting MR scanner was used on 60 HIE cases to conduct the examination,and 10% chloral hydrate was taken orally or enema(0.3~0.5 ml/ kg) 30mintes before tests,in order to complete the inspection during sleep.Routine axial T1WI,T2WI and DWI scan were conducted and axial SWI scan was added.Lesions detection rate of conventional head MRI sequences and SWI sequence,location and nature were observed.ResultsCerebral microbleeds lesions detected by SWI was the most in number,comparing on detection rate,SWI were significantly higher than those of T1WI,T2WI and DWI(P0.01).SWI were significantly superior to T1WI,T2WI and DWI in the lesion size,scope and boundary definition.ConclusionsSWI sequence is better than conventional sequence on displaying the number and scope of small hemorrhage in HIE,and it has obvious advantages on assessing the level and prognosis of HIE.
