异型气管导管、钢丝气管导管及普通气管导管在五官科手术中的应用比较

摘　　要：目的评价3种气管导管在五官科手术中的安全性及实用性。方法择期五官科手术63例,随机分为3组,异型气管导管组(A组,n=25);钢丝气管导管组(B组,n=17);普通气管导管组(C组,n=21)。观察插管前后及术中病人的MBP、HR、SpO2。结果三组之间插管前后比较无明显差异,术中B组出现2例因导管滑入主支气管引起SpO2下降;C组出现1例因导管打折致SpO2急剧下降,需紧急更换异型气管导管。结论与普通气管导管和带钢丝气管导管相比,异型气管导管具有特定的弯曲度,容易固定且价格低廉,在五官科手术中的应用比较完全、实用。Objective To evaluate the safety and practicality of three kinds of tracheal tubes in ENT surgery.Methods 63 elective patients were randomly divided into three groups：shaped endotracheal tube group（A group,n=25）;spiral wire embedded endotracheal tube group（B group,n=17） and common endotracheal tube group（C proup,n=21）.MBP,HR,SpO 2 were observed before and after intubation and during the surgery.Results No significant difference had before and after intubation.During the operation,there were 2 cases of SpO 2 drop caused by tube into main bronchus in B group,1 case of SpO 2 drop rapidly for catheter discount,requiring to change shaped endotra-cheal tube emergently.Conclusions Compared with the common endotracheal tube and spiral wire embedded endotracheal tube,shaped en-dotracheal tube has the advantages of special curvature,easy immobilization,lower cost,which has the reliability and utility in ENT surgery.
