新诊断的2型糖尿病合并非酒精性脂肪性肝病患者血浆内脏脂肪素与胰岛素敏感性的关系

摘　　要：目的探讨新诊断的2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)患者(T2DM+NAFLD组)血浆内脏脂肪素(Visfatin)表达水平及其与胰岛素敏感性(ISI)的关系。方法选取新诊断的T2DM患者94例,其中不伴NAFLD者46例,伴NAFLD者48例。取单纯NAFLD及正常体检者(NC组)各30例作为对照。观察其血浆Visfatin水平并计算其ISI。结果 T2DM+NAFLD组、T2DM组及NAFLD组血浆Visfatin浓度均高于NC组(P<0.05或P<0.01),但糖尿病亚组间比较差异无显著性(P>0.05)。血浆Visfatin浓度与空腹血糖、胰岛素抵抗呈正相关,与ISI呈负相关。结论新诊断的T2DM合并NAFLD者血浆Visfatin水平显著升高,其水平高表达除了类胰岛素样作用外,可能参与胰岛素增敏作用。Objective To explore the level of plasma visfatin in type 2 diabetes with non-alcoholic fatty liver disease（T2DM plus NAFLD） and the relationship between visfatin and insulin sensitivity index （ISI）. Methods Newly diagnosed type 2 diabetes patients（T2DM,n=94） selected,and divided into two groups according to with or without NAFLD. NAFLD without T2DM and healthy individuals （NC） were as controls. The level of plasma visfatin and ISI were observed. Results The level of plasma visfatin was significiantly higher in patients with T2DM plus NAFLD, T2DM, NAFLD than NC（P〈0. 01, P〈 0.01, P〈0. 05）. But there was no significance between these groups. There existed positive correlation of visfatin with fasting plasma glucose（FPG） （r=0. 306,P〈0.01） and insulin resistance（IR） （r=0. 344,P 〈0. 01） ,and a negative correlation between visfatin and ISI （r= 0. 344,P〈0. 01）. Conclusion Visfatin has an insulin-like function and may involve an insulin sensitivity.
