抗疟药包装对疟疾病人治疗效果的干预研究

摘　　要：目的　探讨抗疟药包装对间日疟病人治疗依从的影响。方法　应用前瞻性队列设计的干预研究，阐明改进抗疟药包装和增加用法说明提高病人治疗依从性。将符合观察条件的间日疟病人随机分配入两组队列之一。对照组接受普通纸包或纸袋的包装，将氯喹３ 天和伯喹８ 天疗法的全疗程药分别包成１ 包１ 次分发，无详细用法说明；实验组接受起泡包装的氯喹和伯喹各１盒，在起泡包装背面标示药名和剂量，并配相应小盒，小盒背面标示简明易懂的说明，小盒内插入详细说明书。治毕后第９ 天进行随访调查，并复查疟原虫和测定尿中氯喹的水平。结果　实验组治疗依从率为９６．８９％ （１５６／１６１）；对照组治疗依从率为８３．４４％ （１３６／１６３），两组之间依从率有非常显著差别（Ｐ＜ ０．０１）。对照组未达到足量治疗主要原因为药片丢失和碎裂，占５９．２６％ （１６／２７）。结论　铝箔起泡包装治疗依从率比普通纸包或纸袋高，且分发简便。Aim\ To explore the affection of antimalarial drug packaging on patient compliance.Methods\ An intervention study using a prospective cohort design showed that improved packaging of antimalarial drugs with written instructions,and increased patients compliance.The eligibility patients with P.vivax malaria were randomly assigned to one of two cohorts.The control group received medication in the normal manner of the full course of chloroquine 3 days and primaquine 8 days dispensed in single paper envelope with no written instructions.The experimental group received medication in two separate blister packs,chloroquine and primaquine,each blister contained a single dose.The packet contained simple,clear written instructions on use.On day 9(after start of treament),each patient was followed up.All patient were retested for the presence of malaria parasites,and tested for presence of chloroquine in urine.Results\ 96 89%(156/161) of the experimental and 83 44%(136/163) of the control groups completed the proper treatment reginmen.The results show a statistical difference(P<0 01) in the compliance rates between the two groups.The difference in compliance could also be due to loss or disintegration of the medication due to poor packaging in the control group(59 26%).Conclusion\ The compliance rate of treatment in the blister packing was higher than the normal pack.The blister pack was a more convenient and time-effcient method of dispensing medication and protecting.
