234例急性冠脉综合征的BNP及D-二聚体结果分析

摘　　要：目的分析BNP及D-D在急性冠脉综合征(ACS)的临床应用及评价。方法选取234例ACS患者(观察组)和45例冠脉造影正常者(对照组)的血浆进行BNP、D-D及APTT、PT、FIB测定,并同时进行超声心动图指标检查。结果不稳定型心绞痛组(AMI)和急性心肌梗死组(UAP)BNP、D-D较对照组、稳定型心绞痛组(SAP)显著增高或明显增高(P<0.01或P<0.05);FIB较对照组增高(P<0.05);APTT、PT较对照组明显降低(P<0.05),以上差异均有统计学意义。AMI和UAP患者心功能Ⅱ组的BNP水平显著高于Ⅰ组(P<0.01);AMI、UAP、SAP的Ⅰ组LVEF水平明显高于Ⅱ组(P<0.05)。直线相关分析表明ACS患者BNP与D-D、FIB呈显著正相关(r分别为0.506、0.286,P<0.05),与LVEF呈显著负相关(r为-0.714,P<0.05)。结论联合检测血浆BNP、D-D水平能评价心功能受损程度,有助于ACS的诊断和预后判断;血浆BNP、D-D检测结果与超声心动图对心功能检测结果相符。Objective To analyse and evaluate the clinical applications of Brain Natriuretic Peptide （BNP） and D-dimer（D-D） in patients with Acute Coronary Syndrome.Methods Select 234 cases of ASC （observation group） and 45 patients with normal angiographies （control group） to detect the plasma BNP,D-D and APTT,PT,FIB,with ultrasonic cardiogram as well.Results Compared with control and SAP （Stable Angina Pectoris） groups,BNP and D-D in AMI （Acute Myocardial Infarction） and UAP （Unstable Angina Pectoris） groups were significantly higher （P 0.01 and P 0.05,respectively）.FIB was also significantly higher than control （P 0.05）,while APTT and PT were significantly reduced （P 0.05）.In AMI and UAP patients,BNP level of cardiac function Ⅱ group was significantly higher than Ⅰ group （P 0.01）.In AMI,UAP,and SAP patients,LVEF in Ⅰ group was significantly higher than Ⅱ group （P 0.05）.Linear correlation analysis showed that in ACS patients,BNP was positively correlated with D-D and FIB（r =0.506,0.286 respectively,P 0.05）,and negatively correlated with LVEF （r =-0.714,P 0.05）.Conclusion Combined detection of plasma BNP and D-D levels to evaluate the extent of heart function damage in ASC patients comples with the ultrasonic cardiogram results,and contributes to the diagnosis and prognosis judgement of ACS.
