摘 要:目的:探讨阿卡波糖联合苯磺酸左旋氨氯地平对高血压并糖耐量减低(impaired glucose tolerance,IGT)患者肱-踝脉搏波速度(brachio-ankle pulse wave velocity,baPWV)的影响。方法:选择高血压并IGT患者59例,随机分为苯磺酸左旋氨氯地平组(B组)30例,阿卡波糖加苯磺酸左旋氨氯地平组(C组)29例,对照组30例(A组);观察治疗前后血压、血糖、一氧化氮、内皮素(endothelin,ET)及baPWV的变化。结果:治疗6个月后,B组及C组的血压、ET水平及baPWV较治疗前显著下降(P<0.01),NO水平显著上升(P<0.01),C组的餐后2h血糖(2-hour postprandial blood glucose,2hPG)较治疗前显著下降(P<0.01)。两组治疗后,C组的2hPG、ET水平及baPWV明显低于B组(P<0.05),NO水平高于B组(P<0.05)。结论:阿卡波糖和苯磺酸左旋氨氯地平都可以降低高血压并IGT患者的baPWV,改善大动脉的弹性及顺应性。OBJECTIVE To study effects of acarbose combined with levamlodipine besylate on brachil-ankle pulse wave velocity (baPWV) in hypertensive patients complicated with impaired glucose tolerance (IGT). METHODS A total of 59 hypertensive patients complicated with IGT were selected and randomly divided into 2 groups: 30 cases in levamlodipine besylate group (group B)and 29 cases in acarbose combined with levamlodipine besylate group (group C);30 healthy persons with nor mal blood pressure in normal control group(group A). The levels of blood glucose, nitric oxide (NO), endothelin (ET) and blood pressure, baPWV were measured respectively before and after six months treatment. RESULTS At 6 months after treated with levamlodipine besylate and acarbose combined levamlodipine besylate respectively, levels of blood pressure, ET and baPWV were significantly lowered in group B and group C (all P〈0. 01), while level of NO obviously increased (P〈0. 01 ), and level of 2-hour postprandial blood glucose (2hPG) was obviously lowered compared with that before treatment in group C (P〈0. 01 ). After treatment, levels of 2hPG, ET and baPWV in group C were significantly lower than those in group B (P〈 0. 05), while level of NO was higher than that in group B (P〈0. 05). CONCLUSION Acarbose and levamlodipine besylate can decrease baPWV of IGT patients with hypertension, improve elasticity and compliance of aorta, delay and reverse development of arteriosclerosis in IGT patients at early stage.
