中上胸椎骨折脱位35例临床研究

摘　　要：目的探讨上胸椎骨折脱位的临床特点及手术治疗的方法与术后效果。方法回顾性分析中上胸椎骨折脱位35例患者的临床资料,术前脊髓神经功能按美国脊柱脊髓损伤协会(ASIA)标准分级:A级8例,B级12例,C级9例,D级6例。骨折3例,爆裂骨折18例,骨折脱位8例,爆裂脱位6例。均进行多节段椎弓根螺钉内固定治疗中上胸椎骨折脱位。结果所有患者获随访,时间5～24个月,平均25.5个月,均无术中严重并发症发生。结论对于中上胸椎骨折脱位的治疗,多节段椎弓根螺钉内固定可使伤椎得到满意的复位及固定效果。Objective To explore clinical characteristics of a fracture-dislocation of middle-upper thoracic and surgical method and the postoperative effects.Methods Retrospective analysis of 35 cases of upper thoracic spine fracture and dislocation of the clinical data,preoperative spinal cord nerve function according to American Spinal Injury Association（ASIA）Standard grade：A grade was 8 cases,B grade was 12 cases,C grade was 9 cases,D grade was 6 cases.3 cases of Fracture,18 cases of burst fracture dislocation,8 cases,blowout fracture dislocation 6 cases.8 cases of fracture dislocation,6 cases blow out dislocation.All of cases were treated fracture-dislocation of the thoracic spine by many segment pedicle screws internal fixation.Results All of patients had followed up from 5 to 24 months,which average was 25.5 months.There were no intraoperative serious complications.Conclusion For the treatment of thoracic fracture dislocation began,many segment pedicle screws to injury fixation of vertebral can obtain satisfactory reduction and fixed effectively.
