前后联合入路治疗复杂髋臼骨折

摘　　要：目的探讨经髂腹股沟入路联合Kocher-Langenbeck(K-L)入路治疗复杂髋臼骨折的疗效。方法对2005年1月～2010年4月经前后联合手术入路治疗的46例复杂髋臼骨折进行回顾性分析。结果全部患者均获得随访,平均随访27.2个月,解剖复位31例,满意复位13例,不满意复位2例。髋关节功能:优31例,良9例,可5例,差1例,优良率87%。并发症:坐骨神经损伤1例;异位骨化5例;骨关节炎3例;股骨头缺血性坏死2例;伤口感染1例)。结论前后联合入路具有显露好、复位方便、固定牢靠、异位骨化率低等优点,适用于波及两个柱的复杂的髋臼骨折的治疗。Objective To evaluate the efficacy of surgical treatment of complex acetabular fractures with simultaneous ilioinguinal and Kocher-Langenbeck approaches.Methods A retrospective analysis was conducted on 46 cases of complex acetabular fractures treated surgically with combined approach from Jan.2005 to Apr.2010.Results All cases were followed up for 27.2 months on average.There were 31 cases of anatomic reduction,13 cases of satisfied reduction and 2 cases of unsatisfied reduction.The hip joint function was excellent in 31 cases,good in 9 cases,fair in 5 cases and poor in 1 case,with an excellent rate of 87%.Complications included sciatic nerve injury in 1 case,heterotopic ossification in 5 cases,osteoarthritis in 3 cases,necrosis of the femoral head in 2 cases,and infection in 1 case.Conclusion The combined approaches have the merits of excellent exposure,convenient reduction,stable fixation,and lower heterotopic ossification rate,which are applicable to complex acetabular fractures involved both the anterior and the posterior columns.
