肌腱缝合术中应用生物膜防止肌腱术后黏连的临床研究

摘　　要：目的研究生物膜在肌腱缝合术中防止肌腱术后黏连的作用。方法针对神经修复术后恢复无效的桡神经损伤患者,以是否使用生物膜行肌腱缝合术分为试验组(使用生物膜)和对照组(不使用生物膜)。术后8～24周随访,每1～2个月随访1次。有效随访资料者73例,其中试验组35例,对照组38例。根据中华医学会手外科学会上肢部分功能评定试用标准对两组术后功能分级,并进行两组间卡方检验统计学分析。结果试验组:优4例,良15例,可13例,差3例;优良率54.29%。对照组:优0例,良1例,可14例,差23例;优良率2.63%。可、差组37例再次入院行黏连松解术。结论肌腱缝合术中使用生物膜处理外露肌腱能有效地防止术后肌腱黏连,促进患者术后功能恢复,提高手术疗效,降低再次手术概率,适宜在临床工作中推广应用。Objective To study functions of biomembrane in preventing postoperative adhesion following the tendon suturing.Methods The patients with radial nerve injury who failed in achieving recovery after neural restoration,were grouped into experimental group（using biomembranes） and control group（without using biomembranes）.The long-term follow-up statistics of experimental and control groups were retrospectively analyzed.Patients were followed up in the period of 8-24 weeks after operation,and qualified collection was obtained in 73 patients,including 35 patients in experimental group and 38 patients in control group.According to the functional evaluation standards of upper limb in Hand Surgery Academy of Chinese Medical Association,functional classification and car-square test was performed in the two groups.Results In experimental group,there were 4 cases of excellent,15 cases of good,13 cases of fair,3 cases of poor,with the excellent and good rate of 54.29%;while in control group,there was 0 case of excellent,1 case of good,14 cases of fair,23 cases of poor.All patients in the control group need second operations.Conclusion Using biomembranes in the surgery of tendon suture can effectively prevent the exposed tendons from adhesions,promote functional recovery,improve the curative effect and reduce the probability of re-operation,which is suitable for clinical application.
