摘 要:目的探讨阻塞性睡眠呼吸暂停综合征和高血压靶器官损害的相关性。方法 46例阻塞性睡眠呼吸暂停综合征合并高血压患者设为实验组,29例单纯高血压患者设为对照组。分别监测一般项目、睡眠呼吸暂停指标、心脏结构指标、血管结构指标、肾功能指标、血脂、血糖及心律失常和脑血管意外的发生率,比较两组的差异性。对有统计学意义的靶器官损害指标与睡眠呼吸暂停指标进行多元逐步回归分析。结果实验组与对照组的一般临床资料比较,差异均无统计学意义(P>0.05)。而实验组患者颈围、心脏结构指标〔包括室间隔厚度、左心室舒张末期内径、左室肌重量、左室肌重量指数〕及血管结构指标(颈总动脉内径、颈总动脉内膜中层厚度)均明显高于对照组(P<0.05);实验组患者的颈动脉内斑块、心律失常及脑血管意外的发生率明显高于对照组(P<0.05)。相关分析提示:心脏结构指标与缺氧时间、缺氧程度、缺氧综合指标相关;血管结构指标与缺氧时间指标、缺氧程度指标及缺氧综合指标相关;脑血管意外与缺氧时间、缺氧程度相关;心律失常发生率与缺氧时间、缺氧程度相关(P<0.05)。结论阻塞性睡眠呼吸暂停综合征是引发高血压靶器官损害的重要原因之一。Objective To explore the correlations between obstructive sleep apnea syndrome and damage of the target organs in hypertension.Methods Fourty-six cases of obstructive sleep apnea syndrome complicated with hypertension were enrolled as experimental group and 29 cases of simple hypertension were enrolled as control group.General data,sleep apnea indices,heart structure indices,blood vessel structure indices,renal function indices,blood lipids,blood glucose and the incidence rate of arrythmia and cerebrovascular events were obtained and compared between the two groups.,Stepwise multiple regression analysis were carried out with indices of the target organ damage with significance and sleep apnea indices.Results No significant differences were found in the comparison of general data between the two groups.Neck circumference measures,the heart structure indices(interventricular septal thickness(IVST),left ventricular posterior wall thickness(LVPWT),left ventricle myocardium(LVM),and left ventricle myocardium index(LVMI)),and blood vessel structure indices(common carotid artery internal dimension(CCAId),common carotid artery intima-media thickness(CCA-IMT)) in the experimental group were higher than those in the control group(P0.05).Correlation analysis showed that heart structure indices were correlated with hypoxia duration,hypoxia degree,and integrated indicators of hypoxia;vascular structural indices were correlated with hypoxia duration,hypoxia degree,and integrated indicators of hypoxia;arrhythmia incidence was correlated with hypoxia duration and hypoxia degree.Conclusion Obstructive sleep apnea syndrome is among the utmost important riks factors of target organ damage in hypertension.
