摘  要:目的 研究兔后循环缺血后椎动脉血流参数变化及脑干细胞的形态学改变.方法 将61只8~10月龄日本大耳白兔随机分成正常对照组、椎动脉结扎组和椎动脉狭窄组,分别于术后2 h,1~6 d使用超声测量椎动脉血流动力学参数;取实验兔脑干组织,进行镜下观察.结果 结扎组术后不同时间点椎动脉血流参数与正常对照组比较差异均有统计学意义(P<0.01).狭窄组健侧椎动脉收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)、搏动指数(PI)及阻力指数(RI)与对照组比较明显增高(P<0.05);而狭窄侧PSV、PI、RI及血流量与正常对照组比较明显下降(P<0.05).镜下观察兔脑干细胞缺血2 h时组织结构无明显改变,1 d以上可见脑干区域内部分神经元胞体肿胀、浅染,个别神经元呈细胞凋亡表现.结论 单侧椎动脉狭窄和结扎后,健侧通过内径加宽、PSV加快和血流量增加以代偿椎基底动脉系统的平衡,脑干细胞结构随脑干缺血时间的递增改变越明显.Objective To study the changes of vertebral artery hemodynamic parameters and brain stem cells morphology in rabbits after posterior circulation ischemia. Methods Sixty - one rabbits aged from 8 to 10 monthes were divided into control group, stenosis group and occlusion group randomly. Their vertebral hemodynamic parameters were measured by uhrasonography 2 h, 1 - 6 d after operation, then the brain stem tissue was observed under microscope. Results There was significant difference of vertebral parameters at different time between control group and occlusion groups (P 〈 0.01 ). The peak systolic velocity ( PSV), end - diatolic velocity ( EVD), pluse index (P1) and resistant index (RI) of uninjured side vertebral artery in stenosis group were higher than those in control group( P 〈0.05), while the PSV, PI, RI and blood flow volume were decreased on stenotic side( P 〈0.05). Under the microscope, there were no obvious changes of rabbit brain stem after 2 h of posterior circulation ischemia. After 1 d, swelling, understain or even apoptosis could be seen in some nerve cells. Conclusion After vertebral artery is narrow and occlusive at one side, the other side will compensate to balance the vertebrobasilar system by broadening internal diametre, speeding PSV and increasing volume. The longer the posterior circulation isehemia, the more obvious the struture changes in brain stem cells become.
