摘 要:目的:评价人乳头瘤病毒(human papillomavirus,HPV)基因分型检测在宫颈癌筛查中的临床价值。方法:宫颈癌筛查的妇女1 380例,采用改良导流杂交法行宫颈HPV基因分型检测,分析HPV感染状况、多重感染和基因型别。结果:(1)HPV感染率为32.17%(444/1 380),高危型HPV(HR-HPV)感染率为27.32%(377/1 380),低危型HPV(LR-HPV)为4.86%(67/1380);5种最常见的HR-HPV基因型为HPV-16,-58,-52,-53和-33型,其感染率分别为33.16%(125/377),21.22%(80/377),20.16%(76/377),10.61%(40/377)和9.55%(36/377);(2)≤35岁HPV感染率为26.39%(142/538),>35岁为31.67%(235/742),两组比较有统计学意义(P<0.05);≤35岁HPV双重以上型别感染率为22.50%(32/142),>35岁为19.57%(46/235),两组比较无统计学意义(P>0.05);(3)HR-HPV在正常组、慢性宫颈炎和鳞状上皮化生组、宫颈上皮内瘤变Ⅰ级(CINⅠ)组、CINⅡ组、CINⅢ组和宫颈癌组中的感染率分别为15.0%(157/1 047)、43.41%(56/129)、60.26%(47/78)、86.00%(43/50)、96.36%(53/55)和100.00%(21/21)。结论:HPV基因分型检测可检测出宫颈病变中HPV感染状况及多重感染和基因型别,可作为宫颈癌筛查的初筛手段之一。Objective:To evaluate the clinical significance of human papillomavirus(HPV) detection in cervical cancer screening.Methods:Cervix samples of 1380 women who accepted cervical cancer screening in the first affiliated hospital of Jinan university during January 2007 to December 2010 were collected.Cervix HPV genotyping detection was carried out by using improved HybriMax method.The results were analyzed.Results:HPV infection rate was 32.17%(444/1380)in general.The infection rates of high risk HPV(HR-HPV) and low risk HPV(LR-HPV) were 27.32%(377/1380),and 4.86%(67/1380) respectively.The five common HR-HPV genotypes were HPV-16,-58,-52,-53 and-33,and the infection rates were 33.16%(125/377),21.22%(80/377),20.16%(76/377),10.61%(40/377)and 9.55%(36/377)respectively.The HPV infection rates of patients aged ≤35 and 35 were 26.39% and 31.67%(P0.05) respectively,and the difference was statistically significant.In patients infected by 2 or more types of HPV,the infection rates were 22.50%(32/142) and 19.57%(46/235) respectively for patients that aged ≤35 and 35,and the difference was not significant(P0.05).The HPV infection rates were 15.0%(157/1 047),43.41%(56/129),60.26%(47/78),86.00%(43/50),96.36%(53/55) and 100.00%(21/21) in normal,chronic cervicitis,squamous epithelium metaplasia,gradeⅠ cervical intraepithelial neoplasia(CINⅠ),CINⅡ,CINⅢ and cervical cancer groups.Conclusions:Cervix HPV genotyping detection can provide information of HPV infection status,multiple infection status and HPV genotypes and could be used in cervical cancer screening.
