同期经尿道超脉冲等离子体双极电切联合气压弹道碎石术治疗良性前列腺增生并输尿管结石28例

摘　　要：目的:探讨同期经尿道超脉冲等离子体双极电切联合气压弹道碎石术治疗良性前列腺增生(be-nign prostatic hyperplasia,BPH)并输尿管结石的临床疗效。方法:回顾性分析我院2006年1月～2010年6月接受同期经尿道超脉冲等离子体双极电切联合气压弹道碎石术的良性前列腺增生并输尿管结石患者临床资料。结果:28例患者手术顺利并随访6～59个月,平均13.2个月;国际前列腺症状评分(IPSS)由(23.6±5.4)分下降至(10.1±3.1)分(t=29.316,P=0.000);生活质量评分(QOL)由(4.2±1.0)分下降至(2.5±0.6)分(t=19.442,P=0.000);最大尿流量(Qmax)由(7.9±3.3)mL/s上升至(18.9±6.4)mL/s(t=-18.246,P=0.000);剩余尿量(RU)由(65.1±24.0)mL减少至(22.8±9.0)mL(t=14.884,P=0.000)。结论:同期经尿道超脉冲等离子体双极电切联合气压弹道碎石术治疗良性前列腺增生并输尿管结石,具有安全、并发症少、疗效确切等优点。Objective. To evaluate the clinical efficacy of the simultaneous bipolar plasmakinetic su- perpulse transurethral resection of the prostate and pneumatic lithotripsy on benign prostatic hyperplasia （BPH） complicated with ureteral calculus. Methods： Data of 28 patients with BPH complicated with ure- teral calculus that underwent simultaneous bipolar plasmakinetic superpulse transurethral resection of the prostate and pneumatic lithotripsy from January 2006 to June 2010 were reviewed and evaluated. Results： According to follow up ranging from6 to 59 months（mean, 13.2 months）, procedures were all successful in 28 patients. After surgery, the average IPSSdecreased from （23.6±5.4） to （10.1±3.1） （t=29.316, P=0.000）, Qmaxincreased from （7.9±3.3） mL/s to （18. 9±6. 4）mL/s（t=-18. 246, P=0.000）. The RU volume decreased from （65.1±24.0） mL to （22.8±9.0） mL（t= 14. 884, P=0. 000）, but QOL decreased from （4.2±1.0） to （2.5±0.6） （t=19.442, P=0.000）. Conclusions：With less complications, simultaneous bipolar plasmakinetic superpulse transurethral resection of the prostate and pneumatic litho tripsy is a safe and effective treatment for BPH complicated with ureteral calculus.
