摘 要:目的:观察益肾汤对糖尿病性阴茎勃起功能障碍(ED)大鼠阴茎组织超微结构及神经源性一氧化氮合酶(nNOS)表达水平的影响,探讨其治疗糖尿病性ED的可能机制。方法:取雄性SD大鼠50只,从中随机取10只为正常组,余40只以链脲佐菌素诱导建立DM模型,并行阿朴吗啡阴茎勃起试验筛选糖尿病性ED模型后,随机选20只分为糖尿病性ED组、益肾汤组,每组10只,给药4周后,应用光镜及透射电镜观察各组大鼠阴茎海绵体的形态结构。采用免疫组化SP法检测各组大鼠阴茎组织中nNOS含量。结果:益肾汤组血糖较糖尿病性ED组血糖略下降,但差异无显著性意义(P>0.05);其中糖尿病性ED组大鼠阴茎组织中nNOS表达水平显著低于正常组(P<0.01);益肾汤组nNOS表达水平高于糖尿病性ED组(P<0.01),低于正常组,但与正常组比较,差异无显著性意义(P>0.05)。透射电镜观察:与正常组大鼠比较,糖尿病性ED组大鼠阴茎组织有髓神经纤维排布失序,部分变性、板层分离形成透明空泡或网络状,小血管、内皮细胞、平滑肌细胞和细胞器出现明显损伤;治疗组大鼠阴茎组织有髓神经纤维髓鞘、内皮细胞、平滑肌细胞和细胞器损伤轻微。结论:益肾汤可以显著提高糖尿病性ED大鼠阴茎组织中nNOS表达水平,并改善糖尿病性ED大鼠阴茎组织的损伤。Objective: The effect of Yishen Decoction on penial ultrastructure and neurogenic Nitric Oxide Syntheas(nNOS) expression level in diabetic erectile dysfunction rats was observed for the exploration of its possible therapeutic mechanism. Methods. Fifty male SD rats were used for the experiment. Ten was used for the normal control group, and the other 40 rats were given streptozotocin to induce diabetic erectile dysfunction. We screened diabetic erectile dysfunction models through apomorphine penis erectile test, and then 20 model rats were randomly selected. The 20 model rats were divided into diabetic erectile dysfunction group and Yishen Decoction group, 10 rats in each group. After medication for 4 weeks, we observed the morphological structure of corpus cavernosum penis in rats under light microscope and transmission electron microscope, and detected the nNOS content in penial tissue of rats with Smith-Petersen immunohistochemical method. Results. The blood glucose content in Yishen Decoction group decreased mildly as compared with diabetic erectile dysfunction group, the difference being insignificant (P 〉 0.05). The expression level of nNOS was lower in diabetic erectile dysfunction group than that in the normal control group (P 〈 0.01), and was obviously higher in Yishen Decoction group than that in diabetic erectile dysfunction group (P 〈 0.01). Expression level of nNOS in Yishen Decoction group was lower than that in the normal control group, but the difference being insignificant (P 〉 0.05). The results of examination with transmission electron microscope showed that medullated nerves arranged disorder, and some became denatured, the layers became separated with the formation of transparent bubbles or mesh, and obvious injury was found in small vessels, endothelial cells, smooth muscles and organelles in diabetic erectile dysfunction group. In Yishen Decoction group, the injury of myelinated nerve fiber myelin sheath, endothelial cells, smooth muscles and organelles was mild
