介入放射治疗肛瘘的临床研究

摘　　要：目的　采用放射诊断手段观察肛瘘解剖形态及通流情况，设计介入治疗方法，探索一种治疗肛瘘的新技术。方法　肛瘘患者４１ 例（ 男３５ 例，女６ 例），作者设计介入治疗方法，先从外口插入５Ｆ导管作瘘道造影，探明瘘管的详细解剖情况后，灌入消毒药液进行彻底清洗，再经导管注入硬化剂和栓塞剂（ 鱼肝油酸钠及明胶海绵等） 填塞封闭瘘管。结果　４１ 例中，一次治愈者２９ 例，二次治愈者７ 例，总治愈率９５－１２％ 。结论　本方法治疗肛瘘操作简便，技术先进，无痛苦，无合并症，治愈率高。Objective Intervertional radiology method was designed to treat the anal fistula,that is kept under observing during treatment.Methods 41 patients (35 men and 6 women) with anal fistula or abscess were studied.We performed a fistulography from the external opening to invesitigate the anatomy of anal fistula firstly,then infuse antibacterial solution into the fistula and /or abscess cavity to clean it thoroughly,after that the hardener and embolic agent (sodium morrhuate and potassium aluminum sulfate) were filled into the fistula and closed it.Results Upon 41 patients,29 cases were cured just after first management,and 7 cases were needed the secoud treatment.The total cure rate was 95.12%.Conclusion The interventional therapy is a simple,easy and efficient method for anal fistula.There is little painfulness and no complication.It is especially suit for high anal rectal fistula which is difficult and irritation for surgeons and the low fistulotomy that may cause severe complication and incontinence,this is an updated treatment.
