摘 要:目的:观察烫伤大鼠结肠Kit、Kitl及Gja1基因表达与肠道推进速率的改变,初步探讨严重烫伤后胃肠动力障碍的发生机制及临床意义。方法:56只Sprague-Dawley大鼠随机分为正常对照组(8只),烫伤组(6个时相点各8只,麻醉后背部剃毛造成10cm×7cm的Ⅲ度创面)。烫伤组经乳酸钠林格液复苏后,于伤后3h、6h、9h、16h、24h、48h处死;对照组除烫伤外处理同实验组;处死前30min行碳素墨水灌胃(1ml/只),测量墨水最远端与幽门距离,示为肠道推进速率;另取起始段结肠,提取总RNA后进行实时定量RT-PCR。肠道推进速率及基因2-ΔΔCt值采用方差分析进行统计学处理。结果:伤后各时段大鼠肠道推进速率及Gja1、Kitl mRNA表达显著减弱(P<0.05或0.01);Kit mRNA表达除伤后3h外明显降低(P<0.05或0.01)。结论:烫伤后各时相大鼠肠道运动明显受抑;上述基因表达除3h时Kit外均显著减弱;至48h,各基因表达及肠道推进速率有所回升但仍明显低于正常水平。推测肠组织中Kit、Kitl与Gja1基因表达下调及其后续转录、翻译异常,可能参与了烫伤后胃肠运动障碍的发生过程。To investigate alterations in Kit, Kitl and Gjal gene expressions and to intestinal inter- stitial cells of Cajal (ICC)'s pacemaking activities in scalded rats for further elucidating the mechanism of gas-trointestinal motility disorders after scald. Methods:Fifty-six Sprague-Dawley rats (female, weight 250±30g) were randomly allocated into seven groups, eight for normal control group and forty-eight for six scald groups eight rats were killed (3h, 6h, 9h, 16h, 24h and 48h after burn respectirely). With dorsal hair removed and anesthetized, a 10cm×7cm third degree scald injury was made to the scald groups and sodium lactate Linger's injection was ap- plied soon after the injury. Control group had the same treatment with the exception of scald. All colonic tissues were collected accordingly (control group after twelve hours' starvation). Gastrointestinal transit velocity was eval- uated by intestinal ink propelling distance from the pylorus. Total RNAs were gathered and underwent a real-time quantitative RT-PCR procedure. Ct value of target mRNAs were obtained and 2-△△Ct analyzed via one-way ANO- VA.Results:Both intestinal transit velocities and 2-△△Ct values representing Kit, Kitl and Gjal mRNA levels drop dramatically at all setpoints except 2-△△Ct of Kit 3h post-scald, showing statistic significance when compared with control group (P〈0.05 or 0.01). Conclusion:By the time intestinal motility drops after scald, colonic mRNA ex- pressions of Kit, Kitl and Gjal also confront obvious decreases. Because the vital part of proteins in ICCs' pace- making functioning is originated from above genes, diminished colonic Kit, Kitl and Gjal gene expressions may be to some extent relevant to scalded rats' intestinal motility dysfunction.
