ULTRAMICROSTRUCTURE CHANGES OF SYNOVIUM IN AGED KNEE JOINT OSTEOARTHRITIS

摘  要:目的:通过对老年性膝关节骨关节病患者的滑膜组织进行超微结构的研究,探讨其组织结构方面的变化特征, 为临床工作提供初步的形态学基础。方法:应用光镜及透射电镜观察12 例60 岁以上老年性膝关节骨关节病患者的滑膜组织光镜水平及电镜水平的结构变化。结果:滑膜细胞整体数量水平降低。A 型及B型细胞均出现超微结构方面的改变。粗面内质网膨胀,Golgi氏器减少, 线粒体膨胀溶酶体数量增多, 基质电子密度增加,胶原纤维数量增加, 毛细血管内皮细胞高低不平,管腔被挤向一侧, 腔内可见红细胞,管周可见淋巴细胞及单核细胞浸润。结论:膝关节骨关节病患者滑膜组织均出现超微结构的改变;滑膜组织的变化继发于关节软骨的改变,同时又反过来更进一步加重关节软骨的破坏,使之处于一种恶性循环中;基质中胶原纤维数量增加,基质电子密度的改变继发于滑膜细胞的变化;毛细血管周围淋巴细胞及单核细胞的浸润。Objective: Aged related changes of ultramicrostructures in synovium were studied and mechanism of osteoarthritis due to aging of synovium was approached, so as to provide the morphological basis for treatment. Methods: 12 cases of patients who were aged related knee joint OA were used for this study. The ultramicrostructures of OA synovium were examined under transmission electron microscope. Results: Amount of synovial cell was decrease. Type A and type B cell was noted to be in various degree of ultramicrostructure changes. Dilatation of the cisternae of the RER was a prominent feature in all the cases. Golgi apparatus was decreased. Mitochondria was dilatated. Increase in lysosomal bodies could be seen. Collagen fibers were increased in matrix. Interfibrillar matrix density showed mild increase. There were a lot of lymphocytes and leukocytes impregnation around the capillaries. Conclusion: Synovium tissue in knee joint OA were noted to be in various degree of ultramicrostructure changes; alteration of synovium followed the changes of articular cartilage; changes of matrix electron density and collagen fibers were following the alteration of synovial cells; synovium was revealed only mild nonspecific inflammatory changes in knee joint OA.
