心衰方治疗充血性心力衰竭患者的临床观察

摘　　要：目的观察心衰方对充血性心力衰竭(CHF)患者的临床疗效。方法属NYHA心功能分级为2～3级的门诊及住院患者160例随机分为两组。两组患者均给以病因及对症治疗,对照组按心力衰竭常规处理;治疗组在常规处理的基础上使用心衰方每日一剂分三次服用。观察治疗前后临床症状、体征积分变化;超声心动图指标及运动耐量、生活质量的变化。观察周期共1年。结果 (1)两组患者治疗后症状、体征积分均较治疗前降低,治疗组较对照组积分更低(P<0.05)。(2)治疗组左心室收缩末期容积较治疗前降低,每搏量、有效心输出量、心脏指数、左心室射血分数明显增加与对照组比较差异有统计学意义(p<0.05),提示心脏的收缩功能改善,心排血量增加。与对照组疗效比较差异有统计学意义(p<0.05)。(3)治疗组综合疗效总有效率为94.7%,对照组综合疗效总有效率为75%。组间比较差异有统计学意义(P<0.05)。(4)治疗组的运动耐量、生活质量与对照组比较有较显著提高(p<0.05)。结论心衰方对充血性心力衰竭具有良好防治作用。Objective： To observe on the clinical curative effect of xinshuaifang on congestive heart failure. Methods： 160 patients of the ambulatory treatment or hospitalization with Ⅱ - Ⅲ cardiac function according to the NYHA cardiac function classification are randomly divided into two groups . The two group are given therapies according to their pathogenesis,the control group are given routine medicine treatment ;based on the routine medicine treatment, the treatment group are given Xinshuanfang treatment, a dose three times a day. Before and after treatment, the clinical symptoms, the score of symptoms, changes of Echocardiography index, exercise tolerance and life quality were observed, the observation period is 1 year. Result： （ 1 ） The clinical symptoms and the score of symptoms and signs of the two groups decreased after the treatment , and the treatment group is even lower than the control group （ P 〈 O. 05）. （2） L~ end - distolic Volume （EDV） of the treatment group decreases after the treatment, Stroke Volume （SV） , cardiac output （CO） , cardiac index （CI） , left ventricular ejection fraction （EF） increase significantly, indicating that the myocardial contractility function is improved and the stroke volume is increased, suggesting significant differences when compared with control group （P 〈 0.05 ）. （3） The total therapeutic effective rate of the treatment group is 94.7% and that of the control group is 75%. There are significant differences between the treatment group and the control group（ P 〈 0.05 ）. （4）There are significant differences between the treatment group and the control group in exercise tolerance life quality （ p 〈 0. 05）. Conclusion： Xinshuaifang has evident curative effect on congestive heart failure.
