摘  要:目的了解皖北地区临床分离的耐甲氧西林金葡菌(MRSA)所携带5类药物的7种耐药基因状况,并探讨其耐药特性。方法应用PCR方法检测64株MRSA中mecA、gyrA、qacA/B/C、qacA、ermA/B/C、ermB、tetM耐药基因。结果 64株MRSA中,mecA基因携带率为100%,tetM、gyrA、qacA/B/C、qacA、ermA/B/C、ermB基因携带率分别为95.3%、81.3%、34.4%、15.6%、39.1%、28.1%;基因组合以mecA+tetM+gyrA模式最多,为26.5%。MRSA对万古霉素无耐药,对氯霉素、磷霉素、利福平的耐药性较低,对多种药物表现出较高的耐药性。结论皖北地区MRSA中,mecA、tetM、gyrA基因携带率较高,而qacA/B/C、qacA、ermA/B/C、ermB基因携带率相对较低。Objective The pattern of genes conferring resistance to 5 antibiotics were investigated in clinical methicillin-resistant S.aureus(MRSA) strains isolated in northern Anhui.Methods Polymerase chain reaction(PCR) technique was used to detect the mecA,gyrA,qacA/B/C,qacA,ermA/B/C,ermB,tetM genes in 64 strains of MRSA.Results The mecA gene was identified in all the 64(100%) strains.Genes like tetM,gyrA,qacA/B/C,qacA,ermA/B/C,and ermB were positive in 95.3%,81.3%,34.4%,15.6%,39.1%,and 28.1% of the strains,respectively.The most common(26.5%) pattern of gene combination was mecA+tetM+gyrA.No vancomycin-resistant MRSA was identified.The MRSA strains showed relatively lower resistance to chloramphenicol,fosfomycin,and rifampicin,but higher resistance to other antibiotics tested.Conclusions The prevalence of mecA,tetM,and gyrA genes is high in the MRSA strains isolated from northern Anhui,while the prevalence of qacA/B/C,qacA,ermA/B/C,or ermB gene is relatively lower.
