摘 要:目的探讨高尿酸血症(HUA)患者不同性别对代谢综合征(MS)及其代谢组分的影响。方法采用酶法及酶联免疫吸附试验(ELISA)测定2 140例(男1 314例,女826例)受检者空腹血糖(FBG)、空腹胰岛素(FINS)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)和尿酸(UA),计算胰岛素抵抗指数(HOMA-IR),同时测定受检者腰围、血压。将所有受检者按国际糖尿病联盟(IDF)标准分为非MS组与MS组,按1997年美国风湿病协会标准分为非HUA组与HUA组。结果 MS患病率为15.56%(男性为18.57%,女性为10.77%),HUA患病率为24.25%(男性为27.78%,女性为18.64%)。MS患病率男、女性均随年龄增加而上升;男、女性MS组年龄、腰围、舒张压、收缩压、TG、FPG、FINS、HOMA-IR和UA结果均高于非MS组(P<0.05);HDL-C 2组间差异无统计学意义(P>0.05)。中心肥胖是患MS的风险因素。男性HOMA-IR高于女性。1项、2项代谢紊乱中男性HUA患病率高于女性;3项以上指标代谢紊乱则男、女性HUA患病率趋势相近。结论男、女性患者HUA可以作为MS及MS代谢组分的评价因素。Objective To investigate the influence of hyperuricemia(HUA) in different gender patients on the metabolic syndrome(MS) and its components.Methods The fast blood glucose(FBG),and fast insulin(FINS),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),uric acid(UA) and homeostatic model assessment of insulin resistance(HOMA-IR) were determined by enzymatic test and enzyme-linked immunosorbent assay(ELISA) in 2 140 subjects(1 314 males and 826 females).Their waistline and blood pressure were measured.According to the standard of International Diabetes Federation(IDF),the subjects were classified into nonstandard MS and standard MS groups.According to the standard of American Rheumatism Association,the subjects were classified into non-HUA and HUA groups.Results The prevalence of MS was 15.56%(18.57% in males and 10.77% in females),and the prevalence of HUA was 24.25%(27.78% in males and 18.64% in females).The prevalence of MS in both males and females increased with age.The age,waistline,diastolic pressure,systolic pressure,TG,FPG,FINS,HOMA-IR and UA were higher in MS group than in non-MS group(P0.05).The HDL-C levels in the 2 groups had no statistical difference(P0.05).The central obesity was the common risk factor for MS.The HOMA-IR of males was higher than that of females.The prevalence of HUA in males was higher than that in females for metabolic disorder with 1 and 2 items,while the prevalence was almost the same in both males and females for metabolic disorder with more than 3 items.Conclusions HUA may be an evaluation factor for MS and MS components in both male and female patients.
