血清β-人绒毛膜促性腺激素、孕酮、癌抗原125用于早期异位妊娠诊断的价值

摘　　要：目的:评价血β-人绒毛膜促性腺激素(β-HCG)、孕酮(P)及癌抗原125(CA125)对异位妊娠的诊断价值。方法:回顾性分析我院2007年2月至2010年1月住院拟诊为异位妊娠的患者89例的临床资料,所有患者均于就诊当天检测血β-HCG、P系CA125,48小时后,再次检测血β-HCG,通过资料计算ROC曲线下面积、(AUC)。结果:52例确诊为异位妊娠,12例为宫内早孕,21例为宫内早孕流产,4例为难免流。ROC工作曲线显示,血β-HCG48h/oh的诊断准确度最高(AUC=0.86),其次为P(AUC=0.81),CA125的诊断准确度最低(AUC=0.62)。结论:各检测指标用于诊断异位妊娠的灵敏度和差异度有所差异,在诊断时可结合临床使用。Objective：To evaluate the role of Serumβ-HCG,progesterone（P）and cancer antigen 125（CA125）in the diagnosis of ectopic pregnancy.Methods：The clinical data of 89suspected ectopic pregnancy pregnant women were retrospectively tested in our hospital from FEB 2007to JAN 2010.Serumβ-HCG,P and CA125were measured the same day,After 48h,testing the Serumβ-HCG again,calculating Area Under The Curve（AUC）with data.Results：52cases were diagnosed as ectopic pregnancy,12cases were early pregnancy in uterine,21cases were threatened abortion and 4cases were inevitable abortion.ROC curve showed that the highest accuracy rate in the diagnosis of ectopic pregnancy wasβ-HCG 48h/0h（AUC=0.86）,the second was progesterone（AUC=0.81）,the lowest was CA125（AUC=0.62）.Conclusions：Every index has respective sensitivity and specificity in the diagnosis of ectopic pregnancy,and we can combine Clinical using In diagnosis.
