脑脊液淋巴样细胞检出对结核性脑膜炎早期诊断价值

摘　　要：目的：为了使脑脊液细胞学检查能对结核性脑膜炎提供早期诊断依据。方法：用侯氏法收集脑脊液细胞，对78例结核性脑膜炎与85例其它中枢神经系统感染患者脑脊液细胞学结果进行对比分析。结果：结核性脑膜炎早期脑脊液中淋巴样细胞和浆细胞捡出阳性率分别为95％和62％；而病毒性脑膜炎则为73％和49％、隐球菌性脑膜炎为50％和33％、寄生虫性脑膜炎为56％和对％、化脓性脑膜炎为25％和13％，均明显低于结核性脑膜炎。结论：脑脊液中淋巴样细胞和浆细胞阳性率明显增高是结脑早期的一个重要特征。若能结合生化检查和临床，可为结脑早期诊断提供有力依据。:Objective: To investigate early diagnosis of tuberculous meningitis (TBM ) by cytologicyexamination of cerebrospinal fluid (CSF). Methods: The cells in CSF in 78 patients with TBM and 85 patients withother infections dseases of central nervous system (CNS) in the early stages were collected and examined by usingHou's method. Resuits: The results showed that detection rates of Lymphoid cells and plasmacytes in CSF in theTBM were 95% and 62 % respectively. Howerer,the detection rates in the viral meningitis were 73% and 49%,inthe cryptococcameningitis, 50 % and 33%,in the zoogenic meningitis , 56 % and 3l %,in the suppurative meningits,25% and 13%,respectively. The positve rate of patients with TBM was significantly higher than those of patientswith other infections diseases of CNS. Conclusions:The increase of positive rates of lymphoid cells and plasmacytesis an important feature in patients with TBM in the early stages. Combined the feature with biochemistalexamination of CSF and patient's clinical manifeslations, the early diagnosis of TBM can easily be made.[
