摘  要:目的应用彩色多普勒超声检测子痫前期孕妇肾动脉血流的参数变化特点,评价其临床应用价值。资料与方法选择秦皇岛市妇幼保健院门诊及住院子痫前期患者120例,其中重度子痫前期组62例(A组),轻度子痫前期组58例(B组);应用彩色多普勒超检测两组患者右肾动脉血流参数,并选择同期105例正常孕妇作为对照组(C组)。检测参数包括右肾动脉主干、段动脉、叶间动脉收缩期峰值流速(peaksystolicvelocity,PSV)、舒张末期流速(enddiastolicpeakvelocity,EDV)、S/D值、阻力指数(resistanceindex,RI)、搏动指数(pulsatilityindex,PI)。结果 3组间总体均数差异有统计学意义(P<0.05),其中重度子痫前期组各级肾动脉RI、PI及S/D明显高于轻度子痫组和正常妊娠组(P<0.01),EDV明显低于轻度子痫组和正常妊娠组(P<0.05);轻度子痫前期组各级肾动脉血流参数与正常妊娠孕妇组比较,差异无统计学意义(P>0.05)。结论重度子痫前期患者肾动脉血流舒张期流速明显减低,阻力明显升高,彩色多普勒超声无创、可重复性好,能够准确检测子痫前期患者肾动脉血流动力学变化,有一定的临床应用价值。Purpose To investigate the features of renal artery flow parameters in patients with preeclampsia using color doppler,and to evaluate the clinical utilization.Materials and Methods 120 cases were recruited inlcuding 62 cases with severe preeclampsia(group A) and 58 cases with mild preeclampsia(group B).Another 105 normal pregnancy were enrolled as control group.Color doppler was used to perform flow imaging(CDFI).Flow parameters of right renal artery were peak systolic velocity(PSV),end diastolic peak velocity(EDV),ratio of peak systolic velocity and end diastolic peak velocity(S/D),resistance index(RI) and pulsatility index(PI).Results EDV,S/D,PI and RI in group A were higher than group B and group C with statistical significance(P 〈 0.01).These parameters were not significantly different between group B and group C(P 〉0.05).Conclusions The resistance of renal artery blood flow in severe preeclampsia is higher than mild preeclampsia and normal pregnancy.Color doppler ultrasonography is a noninvasive way to detect the changes of flow parameters in patients with severe preeclampsia.
