摘  要:目的 探讨骨纤维结构不良(FDB)的MRI特征,以提高对该病诊断和鉴别诊断的能力.方法 30例经病理证实的骨纤维结构不良,其中颅面骨11例21个病灶,四肢长骨17例19个病灶,腰椎椎体2例2个病灶,将60%以上病例或病灶所拥有的表现定义为常见表现.结果 (1) 颅面骨FDB常见表现:病变多发(72.73%),好发于额骨(72.73%)与筛骨(63.64%),以实性成分为主(100%),弥漫生长无边界(95.24%),T1WI实性部分等或稍低信号(80.95%);T2WI实性部分均匀等、低信号(71.43%);实性部分中-重度均匀强化(100%);病灶内有小圆形囊变(66.67%)和小片状坏死(90.48%),无高信号出血(76.19%),无软组织肿块及边缘水肿(100%).(2)四肢长骨与腰 椎FDB常见表现:病变单发(89.47%),好发于股骨(63.15%),以实性成分为 主(95.24%),边缘有"低信号环"(100%).T1WI实性部分等或稍低信号(76.19%),T2WI实性部分中等高信号(71.43%),实性部分中-重度较均匀强化(90.48%).病灶内有类圆形或不规则囊变(66.67%)或小片状坏死(90.48%),无高信号出血(100%)及软组织肿块(100%),边缘无水肿(90.48%).结论 四肢长骨与腰椎的FDB表现类似,与颅面骨FDB在病变边界、囊变形态上表现有所不同,其MRI常见表现大部重叠.Objective To study MR imaging findings of fibrous dysplasia of bone(FDB) ,so that to improve the diagnostic accuracy and differential diagnosis of this disease. Methods 30 patients with histologically proven fibrous dysplasia of bone were investigated by MRI (T1WI, T2 WI, intravenous gadolinium administration). The common signs were defined as the signs appeared in 60% or more cases. Results (1) The common signs of FDB in craniofacial bones included:multiple lesions( 72.73 % ) ,located in frontale (72.73 %) and ethmoid(63.64%1 , predominantly solid portion of the lesions ( 100% ) , diffusion growth with indefinitive margins (95.24 % ), small round cystic degeneration (66.67 % ) and small areas of necrosis (90.48 %) inside the lesions, no hyperintense hemorrhage(76.19 % ), without soft tissue masses (100 % ), with no edema around the lesions(100 % ). On MR images, the solid parts of the lesions were isoor slight hypointense on T1WI(80.95 % ), homogeneous isoor hypointense on T2 WI( 71.43 % ), and enchaneed moderately to markedly following contrast medium administration( 100% ). (2) The common signs of FDB in long bones and lumbar vertebral bodies included: single lesion (89.47 %) ,located in femur (63.15 %), with integrated or unintegrated hypointensity margin (100M) , predominantly consist of solid portion(100M), predominantly intermediate or slight hypointensity on T1 WI(76.19% ), predominantly inhomogenerous hyperintensity on T1WI (71.43%), with ellipse folliculus or irregularity (66.67%), small areas of necrosis (90.48%), no hyperintense hemorrhage (100% ), without soft tissue masses ( 100%), with no edema around the lesions (100%). Conclusion The lesions of fibrous dysplasia of bone located in long bone have the similar MRI features to those located in the lumbar spine, but are different from those located in craniofacial bone in lesion border and cyst morphology, most common manifestations of MRI
