改良外侧切口微创内固定治疗跟骨骨折的疗效分析

摘　　要：目的观察和分析改良外侧切口微创手术中撬拨复位及空心钉内固定治疗跟骨骨折的疗效。方法 2007年3月~2009年6月对22例跟骨骨折患者采用跟骨改良外侧切口进行骨折微创复位内固定,根据手术前后X线片对跟骨高度、宽度、长度及跟骨Bohler角和Gissane角进行评估,并观察术后切口愈合情况。采用Creighton-Nebraska评分标准系统评价临床功能疗效。结果所有患者均获得随访,X线片显示术后跟骨高度、宽度、长度及跟骨Bohler角和Gissane角均获得满意恢复,根据Creighton-Nebraska评分标准系统：3足Ⅰ型骨折,平均得分88.6分;12足Ⅱ型骨折,平均得分86.7分;6足Ⅲ型骨折,平均得分85.2分;4足Ⅳ型骨折,平均得分72.6分。术后早期均未出现切口裂开、坏死及感染,手术效果满意。结论采用跟骨改良外侧切口进行骨折微创复位内固定是治疗跟骨骨折的有效方法 ,并可降低术后早期切口并发症的发生。Objective To assess the efficacy of improved lateral microinvasive incision and cannulated screw fixation for fractures of calcaneus.Methods All cases were treated by the improved lateral microinvasive incision and cannulated screw fixation.The efficacy was observed in terms of the height,extent,length,Bohler and Gissane angle of the calcanea befoe and after surgery in X-ray.Simultaneously,the instance of incision was observed.Creighton-Nebraska grade system was used to evaluate clinical function.Results All cases were followed up.The figure,Bohler and Gissane angle of the calcanea got better.According to the Creighton-Nebraska grade system,average score was 88.6 in 3 cases of type Ⅰ,86.7 in 12 cases of type Ⅱ,85.2 in 6 cases of type Ⅲ,and 72.6 in 4 cases of type Ⅳ.No infection and necrosis of skin around the incision occurred.Conclusion Satisfactory outcome can be expected with improved lateral microinvasive incision and cannulated screw fixation for calcaneal fractures.But in type Ⅳ fractures,the result is not as good as type Ⅱ and type Ⅲ.
