原发性骨髓纤维化患者感染的临床特点分析

摘　　要：目的研究原发性骨髓纤维化(IMF)患者感染的临床特点。方法回顾分析对IMF并发感染病例的临床资料,并进行统计分析。结果 48例IMF患者中感染率为45.8%。最常见的感染部位是肺部,占45.5%。感染的绝大多数病原菌为革兰氏阴性(G-)菌,占57.1%。男性患者和女性患者感染率分别为46.2%和45.4%(P>0.05)。老年组(>60岁)患者和非老年组(<60岁)患者感染率分别为69.2%和18.2%(P<0.05)。白细胞数<4.0×109L-1患者和白细胞数>4.0×109L-1患者感染率分别为66.7%和29.6%(P<0.05)。血小板数<20×109L-1患者和血小板数>20×109L-1患者的感染率分别为69.2%和37.1%(P<0.05)。血红蛋白<90 g/L患者和血红蛋白>90 g/L患者感染率分别为58.6%和26.3%(P<0.05)。清蛋白<35 g/L患者和清蛋白>35 g/L患者感染率分别为64.3%和38.2%(P<0.05)。结论高龄、白细胞、血红蛋白及血小板降低可能是IMF患者并发感染的易感因素,积极控制易感因素是预防IMF患者感染的关键措施。Objective To study the clinical features for myelofibrosis with infection.Methods We studied retrospectively the clinical data of MF patients with infection.Results The incidence rate of MF complicated with infection in MF was 45.8%.The most common infectious location was lungs（45.5%）.The majority of disease producing germ was gram-negative bacteria（G-）,with the infection rate of 57.1%.The incidence rate of infection was 46.2% in the male group and 45.4% in the female group（P0.05）.The infection rate was 69.2% in the aged group（ 60 year s）and 18.2% in non-aged group（ 60 years）（P0.05）.The incidence rate of two groups of leucocyte count 4.0×109L-1 and 4.0×109 L-1were respectively 66.7% and 29.6%（P0.05）.The incidence rate were respectively 69.2% in the group of blood platelet count 20×109 L-1 and 37.1% of blood platelet count 20×109 L-1（P0.05）.The incidence rate were respectively 58.6% in the group of hemoglobin content 90 g/L and 26.3% of hemoglobin content 90 g/L（P0.05）.The incidence rate of two groups of albumin count35 g/L and 35 g/L were respectively 64.3% and 38.2%（P0.05）.Conclusion MF complicated with infection has high incidence possible risk factors including eld,decrease of leucocyte,blood platelet and hemoglobin.Positive control of possible risk factors is the important intervention of infection prevention.
