早发心肌梗塞患者健康子代血管内皮功能的检测

摘　　要：目的 :了解早发心肌梗塞家族史对健康子代血管内皮依赖性舒张功能的影响。　　方法 :采用高分辨超声技术 ,检测了 2 0例有早发心肌梗塞家族史的健康子代 (心肌梗塞子代组 )和 2 0例无心血管病家族史对照者 (正常对照组 )的肱动脉在静息状态下、反应性充血及含服硝酸甘油后的内径变化。　　结果 :发现心肌梗塞子代组肱动脉血流介导性舒张功能较正常对照组明显减弱 [(7.8± 4.6 ) %比 (14.9± 3.3) % ,P<0 .0 0 1],而 2组对硝酸甘油 (glyceryl trinitrate)的反应无显著性差异 [(2 4.3± 8.1) %比 (2 5 .2± 4.1) % ,P=0 .186 ]。　　结论 :早发心肌梗塞健康子代可能存在血管内皮依赖性舒张功能障碍。Objective:To investigate whether endothelium dependent vasodilatation is impaired in offspring of patients with premature coronary heart disease. Methods:With high resolution ultrasound,we measured the diameter of the brachial arteries at rest,during reactive hyperemia,and after sublingual glyceryl trinitrate in 40 subjects,consisted of 20 controls with no family history of cardiovascular disease and 20 offsprings of patients with positive family history of premature coronary heart disease. Results:In subjects with positive family history,flow mediated dilatation in arteries was much reduced in comparison with the control group [(7 8±4 6)% vs.(14 9±3 3)%, p <0 001].However,there was no significant difference in response to glyceryl trinitrate between the two groups [(24 3±8 1)% vs.(25 2±4 1)%, p =0 186]. Conclusion:Endothelium dependent vasodilatation is probably impaired in offsprings of patients with premature myocardial infarction.
