摘  要:目的 :观察大剂量氨甲苯酸在体外循环术中对血小板的保护作用。  方法 :2 2例心脏病手术患者随机分为氨甲苯酸组 (n=14)和对照组 (n=8)。氨甲苯酸组于手术开始前静脉滴注和预充液中一次性加入氨甲苯酸各 15 mg/kg(共 30 mg/kg) ,分别于麻醉诱导后、体外循环转流 30分和手术结束时测定血浆凝血烷 (TXB2 )、前列腺素 E2 (PGE2 )、血浆 α颗粒膜蛋白 - 140 (GMP- 140 )的浓度 ,并记录 2 4小时纵隔心包引流量以进行对比。  结果 :对照组测定的 TXB2 、PGE2 、GMP- 140的浓度在转流 30分和手术结束时段显著高于氨甲苯酸组 (P<0 .0 1)术后 2 4小时纵隔心包引流量氨甲苯酸组较对照组减少 (P<0 .0 5 )。  结论 :预防性大剂量氨甲苯酸在体外循环中能有效地保护血小板功能 ,减少术后出血。Objective:To observe the protective effect of high dose P aminomethylbenzoic acid on the platelet function during Cardiopulmonary bypass(CPB). Methods:Twenty two patients were randomly divided into two groups.P aminomethylbenzoic acid group( n =14)received 15mg/kg P aminomethylbenzoic acid before and priming solution of the cardiopulmonary bypass respectively,and 8 patients as control.Thromboxane B 2(TXB 2),Prostagtandin E 2(PGE 2),α granule membrane protein 140(GMP 140)were determined before CPB,at thirty minutes of CPB,at end of CPB respectively,The amount of first 24 hour postoperative blood loss were measured. Results:TXB 2,PGE 2,GMP 140 in the P aminomethylbenzioc acid group was lower than the control group during and end cardiopulmonary bypass ( p <0.01).During the first 24 hour,postoperative blood loss was reduced in the P aminomethylbenzoic acid group as compared with that in the control group(241.21±108.52ml,vs.366 00±196 34 ml, p <0 05). Conclusions:These findings indicated that high dose P aminomethylbenzoic acid administration was effective in reducing postoperative blood loss and preserved the integrity of the platelet during CPB.
