摘 要:目的:探讨臂踝脉搏波传导速度(baPWV)与心血管危险因素及国人缺血性心血管疾病(ICVD)危险评分的相关性。方法:2010年3月~7月天津市人民医院体检人群232例进行常规体检,应用全自动动脉硬化测定仪VBP-9测定双肢的baPWV,按代谢异常组分进行分组,分别比较各组临床基本资料、baPWV及ICVD风险评分。结果:随着代谢异常组分的增加,baPWV逐渐增加,代谢异常各组baPWV均高于对照组,各组间差异均有统计学意义(P〈0.05)。不同组之间,体质量指数、收缩压、舒张压、空腹血糖、三酰甘油、10年风险评估随着代谢异常数目的增高而呈上升趋势,高密度脂蛋白胆固醇(HDL-C)随着代谢异常数目的增加而降低,各组间差异具有统计学意义(P〈0.05),糖尿病史、高血压病史、吸烟史各组间差异有统计学意义(P〈0.05)。Logistic回归分析:baPWV影响因素中年龄OR=1.06295%CI(1.009-1.118)、收缩压OR=1.08595%CI(1.032—1.139)、糖尿病史OR:5.65095%Ct(1.735—18.395);国人ICVD危险评分与baPWV存在回归关系(r=0.531,P〈0.01)。结论:年龄、收缩压、糖尿病史是早期动脉粥样硬化的影响因素,各种主要心血管危险因素与baPWV测定值相关,ICVD评分与baPWV相关。AIM: To study the relationship between brachial-ankle pulse wave velocity (PWV), cardiovascular risk factors and cardiovascular risk scores. METHODS: Physical examination was performed in 232 subjects ( aged from 40 to 90 years) in Tianjin Union Medical Center and baPWV was assessed by VBP-9, a PWV device. The subjects were subsequently grouped and the basic clinical data, baPWV and risk scores of ischemic cerebrovascular diseases (ICVD) were compared among groups. RESULTS: With the increase of abnormal metabolic variables, baPWV increased gradually and was higher than in control group (P 〈 0. 05). In different groups, body mass index, systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose, triglyceride (TG) and risk scores of ICVD were significantly different with the increase of abnormal metabolic variables, whereas HDL-c decreased ( P 〈 0. 05 ). A significant difference was observed between diabetes group, hypertension group and group with smoking history (P 〈 0. 05 ). Logistic regression of risk factors showed a correlation between age ( OR = 1. 062, 95% CI 1. 009 - 1.118), SBP ( OR = 1. 085, 95% CI 1. 032 - 1. 139) and diabetes history ( OR = 5. 650, 95% CI I. 735 - 18. 395 ). Regression correlation was found between risk scores of ICVD and baPWV (b =0. 531; P〈0. 01 ). CONCLUSION: Age, SBP and diabetes history are the risk factors for baPWV. Key words brovascular Cardiovascular risk factors are correlated with baPWV.
