面部痣样病变476例分析

摘　　要：目的:分析色素痣样病变临床和病理之间的关系,为评估病情、制定正确治疗方案提供线索。方法:对476例面部色素痣样病变术前临床诊断与术后组织病理学检查结果进行回顾性分析。结果:所有临床上表现为色素痣样病变的病例中,病理检查确诊为色素痣者共有385例(80.9%),余91例分别是基底细胞上皮瘤13例,脂溢性角化43例,扁平疣19例,表皮囊肿9例,日光性角化5例,黑素棘皮瘤和黑色丘疹性皮肤病各1例。激光等非手术方法治疗178例,其中复发46例(25.8%)。结论:多种疾病可呈色素痣样改变,单凭临床印象难以确诊。术前可采用削取或切取方法获得病理标本,手术切除是治疗色素痣的最佳方法。Objective To investigate the relation between clinic and histopathology in patients with facial nevusoid pigmentosusis,give a guide to evaluate the illness and treat.Methods The data of preoperative clinical diajgnosis and postoperational pathology of 476 patients with nevusoid pigmentosusis were reviewed retrospectively.Results In all patients with nevusoid pigmentosusis,385 case （80.9%） have been diagnosed neveus pigmentosus by pathological examination.the others were 13 basalioma,43 seborrheic keratosis,19 verruca planae,9 epidermal cyst,5 solar keratosis,1 melanoacanthoma and dermatosis papulosa nigra.298 cases were operated.178 cases were treated by nonsurgical methods,for example laser,but 46 cases （25.8%）recured.Conclusion Many diseases could manifest as neveus pigmentosus.It is dificalty to make a definite diagnosis by clinical impression only.The pathological sample should be peeled or excised before treatment.Surgical excision is the best method for theating pigmented nevus.
