血清降钙素原、C-反应蛋白及白介素6联合测定在鉴别细菌和病毒感染中的价值研究

摘　　要：目的 探讨降钙素原（PCT）、C-反应蛋白（CRP）及白介素6（IL-6）在感染性疾病中的诊断、治疗、预后评价作用.方法 应用免疫发光测量法,对494例住院感染患者和128例健康人进行血PCT测定,主要观察细菌及病毒感染患者血中PCT 的情况,并比较同期CRP、IL-6、WBC水平.结果 全身严重细菌感染患者血浆降钙素原水平显著升高,而病毒感染（包括严重感染）患者血中PCT不升高或仅轻微升高,PCT对细菌性感染的阳性预测值、特异性、敏感性均较高于其他各个项目.结论 血清降钙素原、C-反应蛋白及白介素6联合测定可以提高对细菌感染和病毒感染的鉴别效力,同时可有效的指导临床抗生素的使用、评估治疗效果.Objective Procaleitonin （PCT）, C - reactive protein （CRP） and interleukin 6 （IL- 6） in infectious disease diagnosis, trealment and prognosis role. Methods Measured PCT of 494 hospitalized patients infected and 128 healthy individuals, the main observation of bacterial and viral infections of the blood of patients with PCT, and to compare the same period CRP, IL- 6, WBC levels. Results Patients with severe bacterial infection systemic plasma proealeitonin levels were significantly increased, and viral infections （including severe infec- llon）, elevated blood of patients with PCT do not or only slightly elevated, The positive predictive value and specificity Sensitivity of PCT higher than other projects. Conclusion Serum procaleitonin, C - reactive protein and interlenkin - 6 can increase the joint determination of bacterial infection and the identification of the effectiveness of viral infection, and can effectively guide the clinical use of antibiotics to assess the therapeutic effect.
