单用二尖瓣血流图与结合肺静脉血流图评价左心室舒张功能的差别探讨

摘　　要：目的：探讨单用二尖瓣血流图与结合肺静脉血流图评价左心室舒张功能的差别，旨在寻求更敏感的检测方法。方法：二尖瓣血流图未显示明显异常的Ⅲ期高血压病患者２０例，对照组选择３３例健康志愿者；使用美国惠普公司ＨＰＳＯＮＯＳ２５００彩色超声诊断仪，呼气末记录二尖瓣血流频谱和肺静脉血流频谱；运用ｔ检验进行高血压病组和对照组间二尖瓣血流图和肺静脉血流图各个观察指标的显著性检验。结果：肺静脉血流图显示：①高血压病组与健康对照组相比Ｓ波变小（Ｐ＜０．０１），Ｄ波相对变大，Ｓ／Ｄ＜１；②高血压病组ＡＲＶ（ｃｍ／ｓ）为３９．２６±９．０３与健康对照组（３１．３５±５．００）相比有显著性差异（Ｐ＜０．０１）。结论：应用肺静脉血流图结合二尖瓣血流图评价左心室舒张功能。Objective: To seek for a new EchoDoppler method which can detect the change of left ventricular diastolic function more sensitively.WTHZ]Method: 20 patients with hypertension stage(hypertension group) and 33 normal people(controls) were investigated.Mitral valve flow graph and pulmonary vein flow graph were calculated by EchoDoppler in both groups. Results: Mitral valve flow graph in patients with hypertension showed normal,while pulmonary vein flow graph in these patients showed small S and large D waves.In these patients,ratio of Vs/Vd decreased smaller than 1 and S wave significantly decreased compared with controls(P<0.01).ARV(cm/s) in hypertension group increased compared with controls(39.269.03 versus 31.355.00,P<0.01).HZ]Conclusion: Combining pulmonary vein flow graph with mitral valve flow graph by EchoDoppler to evaluate the change of left ventricular diastolic function is a more sensitive method.
