血浆(1-3)-β-D-葡聚糖及内毒素检测在重症监护病人早期感染诊断中的应用

摘　　要：目的:探讨血浆(1-3)-β-D-葡聚糖及内毒素检测在重症监护病人感染早期诊断中的临床意义。方法:根据体液的真菌培养及血培养结果将患者分为2组,比较2组中血浆(1-3)-β-D-葡聚糖及内毒素定量水平差异。结果:真菌培养阳性患者血浆中的(1-3)-β-D-葡聚糖定量水平(65.20±24.55)pg/ml明显高于真菌培养阴性组(23.15±11.07)pg/ml,且(1-3)-β-D-葡聚糖检测法阳性率为58.93%,高于真菌培养法的48.21%,具有统计学差异(P<0.05);同时血培养阳性患者血浆中的内毒素定量水平(88.50±26.85)pg/ml明显高于血培养阴性组(14.50±2.35)pg/ml,且内毒素检测法阳性率为57.14%,高于血培养法的35.71%,具有统计学差异(P<0.05)。结论:血浆(1-3)-β-D-葡聚糖及内毒素定量测定具有快速、敏感的特点,对真菌及革兰阴性菌感染的早期诊断具有一定的临床应用价值。Objective：To explore the clinical significance of serum（1-3）-β-D-glucan and endotoxin on diagnosis of early infection in the intensive care patient.Methods： Compared the quantitative level of serum（1-3）-β-D-glucan and endotoxin in the two groups which was divided according to the results of fluid fungal culture and blood culture.Results： Quantitative level of the（1-3）-β-D-glucan of fungal culture-positive group（65.20±24.55）pg/ml was significantly higher than that of fungal culture-negative group（23.15±11.07）pg/ml,and positive rate of the（1-3）-β-D-glucan assay（58.93%） was higher than the fungal culture（48.21%）,with significant difference（P0.05）;Meanwhile,quantitative level of the endotoxin of blood culture-positive group（88.50±26.85）pg/ml was significantly higher than that of blood culture-negative group（14.50±2.35）pg/ml,and positive rate of the endotoxin assay（57.14%） was higher than the blood culture（35.71%）,with significant difference（P0.05）.Conclusion： The determination of serum（1-3）-β-D-glucan and endotoxin have some clinical value on the early diagnosis of fungi and gram-negative bacterial,for its advantage of speediness and sensitiveness.
