实时三维超声心动图定量评价冠心病患者PCI术前后左心室重构及逆转

摘　　要：目的探讨实时三维超声心动图(RT3DE)评价冠心病患者冠状动脉介入治疗(PCI)术前后左心室构型变化的价值。方法将57例研究对象分为3组:正常对照组(A组)、急性心肌梗死组(B组)、陈旧性心肌梗死组(C组)。应用RT3DE对CAD患者术前,术后1个月及对照组进行检查,脱机软件分析得出左心室射血分数(LVEF)、舒张末容积(LVEDV)、峰值射血率(PER)、峰值充盈率(PFR),达到最小收缩容积时间的标准差及最大差值(Tmsv16-SD、Tmsv16-Dif)等参数,并将测值分别比较。结果 B组手术前各参数与A组比较差异具有统计学意义(P<0.05),术后各参数与A组比较无明显统计学意义(P>0.05),C组手术前后各参数与A组比较都具有统计学意义,术前比较(P<0.01),术后比较(P<0.05);B、C组各参数手术前后比较差异有统计学意义(P<0.05)。结论冠心病患者左心室发生了不同程度的重构,PCI治疗可使此类重构逆转,RT3DE为CAD患者PCI治疗术前后左心室构型变化的准确评价方法。Objective To assess the value of real-time three-dimensional echocardiography（RT3DE） in evaluation of the left ventricular configuration in patients with coronary artery disease（CAD） pre-and postpercutaneous coro nary intervention（PCI）. Methods Fifty-seven patients were divided into normal control（Group A）,acute myocardial infarction（Group 13）,and old myocardial infarction（Group C）. A series of left ventricular indices by RT3DE examination were analyzed offline pre- and 1 month post- PCI operation,including left ventricular ejection fraction（EF）, enddiastolic volume（EDV）, the peak ejection rate （PER）, peak filling rate （PFR）, the maximal difference of time and standard deviation to the point of minimal systolic volume（Tmsv） of 16 segments（Tmsvl6-SD,Tmsvl6-Dif）. Results There were significant differences of all pre-PCI parameters between group A and Group B（P〈0.05）, but no significant differences post-PCI（P〈0.05）. There were significant difference between pre-（P〈0.01） and post-（P〈0.05） PCI in Group C There were significant differences between pre- and post- PCI in Group B and C（P〈0.05）. Conclusions Left ventricular remodeling could be found in patients with CAD. PCI might reverse left ventricular remodeling somewhat. RT3DE could be a new valuable method of evaluating left ventricular configuration in patients with CAD pre- and post- PCI.
