达肝素钠联合阿托伐他汀钙治疗不稳定型心绞痛临床分析

摘　　要：目的探讨达肝素钠联合应用阿托伐他汀治疗不稳定型心绞痛的临床疗效。方法将126例不稳定型心绞痛患者随机分为观察组和对照组各63例。观察组在对照组用药基础上给予给予达肝素钠5 000 U皮下注射,每12 h 1次,连用7 d。同时给予阿托伐他汀60 mg嚼服(刚入院时),此后每晚睡前口服20 mg,连续服用两周,14 d一个疗程。检测两组患者治疗前后心电图的变化及血脂的测定,在治疗过程中要注意患者皮肤黏膜及内脏是否有出血情况,观察其安全性。结果观察组的总有效率为92.19%,对照组的总有效率为79.03%,两组比较差异有统计学意义(P<0.05)。同时,观察组治疗后总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)均较其治疗前有明显下降,差异有统计学意义(P<0.01或P<0.05),而对照组治疗前后血脂的各项观察指标比较,差异无统计学意义(P>0.05)。结论达肝素钠联合阿托伐他汀治疗不稳定型心绞痛疗效显著,无不良反应,值得推广应用。Objective To explore the efficacy of combination treatment of dalteparin sodium and atorvastatin on unstable angina pectoris（UA）.Methods A total of 126 patients with UA were randomly assigned into treatment and control group（63 for each）.The treatment group was given Dalteparin Sodium 5000 U（sc,q12h×7d） and Atorvastatin（60mg chewing at first time,then 20mg oral administration every night） and the control group was treated with Atorvastatin only.The effecacy of angina treatment and blood lipid changes were evaluated after 2-week treatment for both groups.Results The effective rates of treatment were 92.19% and 79.03% respectively for patients in the treatment and the control groups.The difference of treatment effecacies was significant（P〈0.05）.The concentrations of cholesterol（TC）,triacylglycerols（TG） and low-density lipoprotein cholesterol（LDL-C） decreased significantly（from 6.97±0.71 mmol/L,2.48±0.49 mmol/L and 3.71±0.45 mmol/L to 4.26±0.53 mmol/L,1.39±0.12 mmol/L and 1.82±0.59mmol/L,respectively） on average in the patients with combination treatment（P〈0.01 or P〈0.05）.There were no significant changes in the above blood lipid indexes in the patients of the control group.Conclusion The combination treatment of dalteparin sodium and atorvastatin achieved better effecacies than the single application of Atorvastatin did on patients with UA.
