摘 要:目的运用二维斑点追踪成像(2D-STI)评价正常人左心室心肌应变。方法选择50例正常人,其中男性31例,女性19例;年龄48-54岁,平均年龄51.16岁。收缩压为(15.63±1.77)kPa[(117.53±13.30)mmHg],舒张压为(9.87±1.04)kPa[(74.18±7.84)mmHg];心率(76.43±5.51)次/分。采集左心室长轴和短轴方向的二维动态图像,测量分析心肌收缩期峰值纵向应变(LS)、径向应变(RS)和圆周应变(CS)。结果左心室各节段心内膜层心肌LS、RS、CS均显著高于心外膜层心肌(P〈0.05)。LS由基底部到心尖部递增;RS在同一水平不同节段分布较一致;同一水平前室间隔的CS较高。结论 2D-STI可以准确测量左心室壁节段心肌应变,能够定量评价局部心肌收缩功能。Objective To evaluate the characters of the peak systolic strain for left myocardium in healthy subjects by two-dimensional speckle tracking(2D-STI).Methods A total of 50 healthy volunteers were enrolled,male 31,female 19,aged 48-54 years old,mean aged 51.16 years old.The systolic blood pressure was(15.63 ± 1.77) kPa[(117.53 ± 13.30) mmHg],diastolic blood pressure was(9.87 ± 1.04) kPa[(74.18 ± 7.84) mmHg],heart rate was(76.43 ± 5.51 beats/min).The high frame rate two-dimensional images of the left ventricular long and short-axis view were recorded,and the peak systolic radial strain(RS),circumferential strain(CS) and longitudinal strain(LS) were measured by two-dimensional strain software.Results The LS,RS and CS values of the inner layers in corresponding segments at different levels were higher than those of outer layers(P 0.05).The LS values gradually increased from base to apex(P 0.05).The RS values of different segments at the same level had no significant difference.The CS value of anteroseptal at level of apex was higher than those of the other segments.Conclusion It is demonstrated that the 2D-STI have great values in quantitative analysis regional wall function of left ventricular because it could measure 2D straining value exactly.
