B超在早期宫角妊娠诊治中的临床应用价值

摘　　要：目的探讨B超在早期宫角妊娠诊治中的临床应用价值。方法选择川北医学院附属医院2005年6月至2010年6月临床拟诊早孕经B超诊断的早期宫角妊娠自愿终止妊娠256例患者，年龄21—42岁。做二维或三维超声诊断，并在B超监测引导下施行人工流产或清宫术，全部病例术后10d左右返院复诊。结果全部病例经腹或经阴道B超（包括二维及三维）诊断，其中孕囊型179例。包块型77例；孕囊或包块位于宫角部72例，偏于宫角部184例。B超监测引导下施行人工流产或清宫术，孕囊或包块偏于宫角部184例中，术后复诊行二次清宫术3例（1．63％）；孕囊或包块位于宫角部72例中，术后复诊行二次清宫术13例（18．1％）；1例术中子宫穿孔经腹修补手术治疗。结论B超检查是诊断早期宫角妊娠的最佳方法。可作为首选．三维冠状面成像较二维更准确，对临床预测妊娠预后及选择治疗方法有重要意义。B超监测引导下对早期宫角妊娠施行人工流产、清宫术较之于经腹手术更简便经济．同时是一种安全有效的方法，值得临床推广应用。Objective To investigate the clinical application value of B uhrasound in the diagnosis and treatment of early cornual pregnancy. Methods A total of 256 patients with voluntary termination of early cornual pregnancy from June 2005 to June 2010 were enrolled, which aged 21 - 42 years old. The two-dimensional or three-dimensional ultrasound diagnosis were used, and the monitored B ultrasound-induced abortion of curettage were performed. All patients were requested further consultation about 10 days after surgery. Results All cases were diagnosed by transabdominal or transvaginal, two-dimensional and three-dimensional B ultrasound. The 179 cases were gestational sac type, while 77 cases were mass type. The 72 cases gestational sac or mass were located at cornua uteri and 184 cases were near to cornua uteri. In B ultrasound -induced abortion or curettage, there were 3 cases（1.63 %） for further curettage twice among the 184 cases neared to cornua uteri, and there were 13 cases（18.1%） for further curettage twice among the 72 cases located at cornua uteri. One case with uteri perforation was treated with transabdominal repair surgery. Conclusion It is demonstrated that B ultrasound is the best method and should be the first choice in diagnosis of early cornual pregnancy. The three-dimensional coronal plane imaging is more accurate than two-dimensional imaging and is of great significance in predicting pregnancy prognosis and choosing treatment method. The abortion or curettage induced under B ultrasound monitoring for termination of early cornual pregnancy is simple and economic as well as safe and effective, and which is deserved extended clinical application.
