31例糖尿病水疱病治疗的临床观察

摘　　要：目的探讨中医对糖尿病水泡病的发病原因和病理机制的认识,确定中医药治疗该病的用药思路。方法对2007年6月-2010年5月期间本院住院患者中出现的31例糖尿病水泡病患者,采集其病史、临床体征以及血糖、肾功能、血象等实验室指标。治疗上在全身采取控制血糖、预防感染、改善微循环、活血、通络、补肾的综合治疗基础上,局部应用大黄粉涂撒创面并以黄连膏换药的中药外治法,以及内分泌科常规护理与心理护理,观察临床疗效,对所有的资料进行回顾性总结。结果经过上述治疗,31例患者均痊愈,痊愈最短的时间为治疗后8 d,最长为治疗后11 d,平均为（9.6±0.33）d,均未留下疤痕。结论糖尿病出现水泡病,主要病机为久病及络,络脉瘀阻,络瘀化毒,聚血为液。所以中医药治疗在采取活血、通络、补肾的基础上,局部应用解毒敛疮、去腐生新的治疗思路是有效的。Objective To explore the morbidity reason and mechanism on diabetic bollous disease in Chinese medicine and determine the ideals on the treatment of Chinese medicine for the disease.Methods The laboratory indexes such as medical history,clinical physical signs,blood glucose,kidney function and hemogram were collected in 31 cases of diabetic bollous disease among the inpatients of the hospital from June 2007 to May 2010.The comprehensive treatment was administered for systematically controlling blood glucose,preventing from infection,improving microcirculation,activating blood circulation,promoting the circulation in collaterals and replenishing the kidney.Additionally,the external applications of herbal medicines such as the powder of rheum officinale on the local wound and dressing with coptis ointment,as well as conventional nursing in endocrinal department and psychiatric care were provided.The clinical efficacy was observed and all the data were summarized retrospectively.Results After the above treatment,all 31 cases were cured.The time of treatment was ranged from 8 days to 11 days,averagely(9.6±0.33)days.No scars were left.Conclusion Bollous disease of diabetes is mainly caused by blood stagnation in collaterals and transformation to the toxin in pathogenesis.It is effective to treat the disease with the local application of herbal medicine for counteracting toxin,removing boils and promoting the wound healing on the basis of activating blood circulation,promoting the circulation in collaterals and replenishing the kidney.
